I still remember being 42 and standing in the bathroom, staring at my own reflection… and genuinely wondering, “Where did I go? And how could he possibly still see me when I couldn’t even recognize myself?” That year was the thick of perimenopause for me. Everything felt off. My moods were unpredictable, my patience was thin, my body felt foreign, and my stress threshold dropped to almost nothing. But the hardest part wasn’t the symptoms. It was trying to explain what was happening inside of me when I didn’t understand it myself. I could see my husband trying — watching me, worrying about me, tip-toeing around the shifts he couldn’t make sense of. And all I could think was, “How do I tell him what I’m feeling… when I don’t even have the words?” Here are 10 things I wish I could have told him👇🏼 1. My mood swings aren’t caused by you they’re happening inside my body, often before I even realize it and I don’t know how to control them 2.When I go quiet or pull back, it isn’t loss of love it’s overwhelm and exhaustion. 3.I haven’t lost interest in you …I’m struggling to recognize myself again. 4.Some days closeness feels comforting, other days it feels like too much and I can’t always predict which and I know that is hard for you. 5. I don’t always have words for what I’m feeling, and that’s frustrating for me too. 6.Please don’t try to fix this for me. When you’re calm and present, it actually helps me more than anything. 7.When I say “I’m not okay,” it isn’t about you it’s me trying to say how I feel 8. I can be grateful for our life and still feel like I’m struggling inside it. 9.I don’t need judgment right now, I need understanding, even when this is confusing. 10.I’m still here — I’m just navigating something new. ‼️This season isn’t about blame. So I’m one week into HRT. I was started on an oestrogen patch and progesterone tablets by my GP. Yesterday I put on my 3rd patch. 1 week in and I honestly am seeing a difference. Now I’ll caveat that by saying I’m solo parenting since I started. My husband had to go to the UK and as many of you perimenopausal women will understand, he annoys me like nothing else with the hormonal changes 🤣🤣 (sorry @rossshepherd 🥰🥰 )so it could be no husband or the week of HRT (who knows 😬), but I am definitely less irritable and I haven’t flown off the handle at the kids like I normally would. For me the big issue was emotional lability probably. I was always so easy going but the last few years I would fly off the handle at the slightest thing whether it was Ross or the kids. And that really wasn’t how I wanted to live my life, or parent my children. Parenting is hard, but god this was harder. Coupled with the sleeplessness and the anxiety and honestly I don’t know how I was coping. Queue hormone replacement therapy. I actually think I noticed a difference within 24 hours of the patch going on. And I slept for the first time properly in years: so much so that I was able to turn back over in the AM. Absolutely amazing feeling, but in reality when solo parenting not so much so and we were late for school a few days. My sleep has honestly been terrible for the last few years and I didn’t realise it was hormones at all. My poor husband as well suffered the brunt of a lot of it. Someone in my DMs said it was either get a divorce or HRT and I have no doubt that is how a lot of us are feeling. Now HRT isn’t a one size fits all. It’s a complicated prescription and there are lots of different options to suit everyone. If you’re feeling similar to me speak to your GP or women’s health specialist to get help. I’m sharing my journey as I realise so many of us are going through it. I don’t think a post has ever resonated with people as much. I will share more knowledge on HRT as I go, but I need to remind myself of all the guidance as well. So stay tuned for that. Let me know if you are suffering with any of the above below. It starts with subtle symptoms your labs are already whispering about.” If you’re dealing with things like: - post-meal exhaustion - bloating after fatty foods - histamine reactions - stubborn belly weight - worsening PMS or perimenopause symptoms - low tolerance to alcohol or supplement Your liver may be congested long before anyone calls it “fatty liver.” Early lab markers I watch closely: ⚠️ AST or ALT > 25 ⚠️ GGT > 30 ⚠️ Triglycerides > 100 ⚠️ A1C > 5.5% ⚠️ Rising fasting insulin (over 8-10) ⚠️ Ferritin elevated (above 110) with normal iron & hemoglobin These don’t mean failure. They mean opportunity for early intervention. Because your liver controls: - blood sugar regulation - fat metabolism - estrogen & hormone clearance - histamine breakdown - bile production - detoxification And when it slows down, everything backs up. Simple ways to start supporting your liver: ✔️ Eat protein first at meals (stabilizes glucose + reduces fat overload) ✔️ Limit liquid calories & fructose-heavy drinks ✔️ Reduce ultra-processed oils and excess alcohol ✔️ Prioritize bitter foods: arugula, dandelion, radicchio, lemon ✔️ Walk after meals (improves hepatic insulin sensitivity) ✔️ Support bile flow: adequate hydration, fiber, taurine, phosphatidylcholine\ ✔️ Support glutathione: NAC, vitamin C, sulfur-rich foods ✔️ Address gut dysbiosis (because bile + gut health are inseparable) Fatty liver isn’t just about fat. Most women are told to only think about HRT if their symptoms are “bad enough.” The truth is that HRT is about so much more than hot flashes or night sweats.  When we support our hormones, we are also protecting our bones, our brain, our heart, and our overall quality of life in the years ahead. These benefits are long-term and preventative, which means they are working quietly in the background even if you are not feeling daily symptoms right now. Bone density, memory, cognition, mood, heart health, vaginal and urinary health… all of these are influenced by estrogen, progesterone, and testosterone. That is why HRT is an important piece of menocare, even when symptoms seem mild or manageable. If your doctor has never talked to you about these benefits, it is time to start asking questions. I’m an MD and this is what keeps me up at night about GLP-1 medications. Not the medication itself. The medication is evidence-based, well-researched, and when used appropriately, incredibly effective for metabolic health in perimenopause and menopause, especially in conjunction with HRT the research is starting to say. What keeps me up is watching women use it wrong. Or watching them not use it at all because they’re too scared to ask — even when it could legitimately help them. The problem is the conversation. It’s dominated by two extremes: One side: Influencers using dangerous doses for aesthetics. Little to no medical supervision (and if they are being supervised, shame on that provider - there I said it). Little to no lifestyle foundation. Just rapid weight loss and Instagram photos. They’re risking muscle loss, nutrient deficiency, and metabolic damage. That’s not health. That’s aesthetic chasing. The other side: People calling it “cheating” or “the easy way out.” Shaming women for considering medical interventions. Acting like using evidence-based medicine for metabolic health is somehow morally inferior to white-knuckling your way through the medical conditions of midlife. And women in the middle — dealing with weight gain, insulin resistance, metabolic shifts, exhaustion — don’t know who to believe. They’re confused. They’re scared to ask their doctors. They’re judged either way. Here’s the truth: when GLP-1 medications are used as part of a doctor-supervised protocol that includes strength training, adequate protein, hormone support, and lifestyle foundations — that’s medicine. That’s addressing insulin resistance at the root. That’s working with your body, not against it. The medication isn’t the problem. The lack of honest medical education is. Women deserve better than the extremes. They deserve the actual medical conversation. Follow if you want that version — not the influencer version or the shame version.
