It is a minimally invasive heart procedure used to treat mitral stenosis, a condition where the heart's mitral valve becomes narrowed and restricted, usually due to past rheumatic fever.
Instead of open-heart surgery, a doctor uses a balloon to "crack open" the narrowed valve and restore proper blood flow.
🔧The Procedure:
The entire process usually takes about 1 to 2 hours and is performed in a Cardiac Catheterization Lab (Cath Lab).
📌1. Preparation and Access
Anesthesia: You are typically given local anesthesia at the groin and a sedative to help you relax.
Entry: The cardiologist inserts a small tube (catheter) into the femoral vein in your groin.
📌2. Reaching the Heart
Navigating to the Right Atrium: The catheter is guided up through the vein into the right side of the heart (Right Atrium).
The "Septal Puncture": Since the mitral valve is on the left side of the heart, the doctor must cross over. They use a special needle to make a tiny, precise hole in the wall (septum) that separates the right and left chambers.
📌3. Opening the Valve
Positioning the Balloon: A specialized balloon (most commonly the Inoue balloon) is guided through that hole and positioned directly inside the narrowed mitral valve.
Inflation: The doctor inflates the balloon. This pressure "splits" the fused edges of the valve (the commissures) that were stuck together.
Verification: Using X-ray (fluoroscopy) and ultrasound (Echocardiogram), the doctor checks to ensure the valve is opening wide enough and that there isn't too much leaking.
📌4. Completion
Once the valve is sufficiently opened, the balloon is deflated and removed.
The tiny hole in the heart wall usually heals on its own.
Pressure is applied to the groin to prevent bleeding, and you will typically need to lie flat for several hours

Here's what a Watchman FLX implant would look like inside the heart. It is used to occlude the left atrial appendage as an alternative to anticoagulation in afib for stroke reduction. There is always a learning curve when folks learn procedures like this!
We generally see and learn everything in 2D X-ray as interventionalists, learning procedures using 3D printed models can be super helpful! Knowing what our movements translate into real time can help us better understand what is going on love X-ray and TEE imaging 🫀
