For those who are curious: The control rods at the Chernobyl Nuclear Power Plant played a critical role in the 1986 accident. Their primary function was to slow down or completely stop nuclear fission in the reactor. Control rods are made from neutron-absorbing materials such as boron, silver, and indium, and as they are inserted into the reactor core, they suppress the chain reaction. The control rods in the RBMK-type reactors used at Chernobyl had a serious design flaw. The tips of the rods were not neutron absorbers, but made of graphite. Graphite is a material that slows down neutrons, increasing the probability of fission. Therefore, when the control rods first entered the core, instead of calming the reactor, they briefly increased reactivity. During the 1986 experiment, when the emergency shutdown system was activated, the control rods simultaneously began descending into the core. However, because the graphite tips entered the core first, a sudden and very powerful surge of power occurred at the bottom of the reactor. This unexpected surge became uncontrollable within seconds, triggering the process that led to the reactor's explosion.Furthermore, it took a considerable amount of time for the control rods to fully enter the core in RBMK reactors. This slowness made it difficult to quickly shut down the reactor in an emergency. Both the graphite tip design and the mechanical delay transformed the control rods from a safety device into a significant part of the disaster in the Chernobyl accident. Following the accident, the design of the control rods in RBMK reactors was modified, the graphite tips were removed, and the emergency shutdown systems were accelerated. In normal human terms, it’s like if you were holding a bunch of loose papers and a car went speeding by you - the wind from the car may cause you to drop some of those papers! It’s the same case here. The charged particles from the pulse disrupt the molecules in the water to release photons, or light particles. A nuclear reactor is started up by initiating and carefully controlling a nuclear chain reaction, a process known as achieving criticality. This process is centered on nuclear fission, where a neutron strikes a Uranium-235 atom, causing it to split, release energy (heat), and eject more neutrons that can go on to split other atoms. Startup Procedure The startup begins with the reactor core fully loaded with fuel assemblies, which contain uranium pellets. Control rods, which are made of neutron-absorbing material like boron, are initially fully inserted into the core to prevent any uncontrolled reaction. To begin the fission chain, a source of neutrons is required. In a brand-new reactor with fresh fuel, a startup neutron source (like Californium-252) is often inserted to provide the initial neutrons. In an operating reactor, enough residual neutron activity remains in the partially spent fuel to start the reaction. The reactor is then brought to power by slowly withdrawing the control rods. As the rods are removed, fewer neutrons are absorbed, making more available to sustain the chain reaction. Once enough neutrons are available to keep the fission going on its own, the reactor achieves criticality, and the power level and heat production begin to rise exponentially. Operators then regulate the power output by finely adjusting the control rod positions, ensuring the reaction remains stable and controlled. The heat generated is then transferred to a coolant (often water), which is used to produce steam to spin a turbine and generate electricity.

Unlike previous models, this machine relied entirely on software rather than physical hardware interlocks to prevent radiation overdoses. The fatal error was a “race condition”—a specific software glitch where the machine would trigger if an operator typed commands too quickly. When this happened, the machine would fire a high-power electron beam without the necessary lead shield in place, delivering a massive, invisible dose of radiation directly into the patient’s body. In one instance at the East Texas Cancer Center, a patient felt a “jolt of electricity” and heard a buzzing sound, while the computer screen simply displayed the message “Malfunction 54”. The patients suffered agonizing radiation burns that destroyed their internal organs over several months, resulting in at least five deaths. The manufacturer, Atomic Energy of Canada Limited, initially denied the machine could malfunction, blaming operator error instead. As of 2025, the Therac-25 case remains the most famous example of “Software Horror” in history and is a mandatory study for every medical software engineer to ensure that code never again has the power to silently delete a human life.
