De korte video’s, meestal ongeveer dertig seconden lang, volgen een vast patroon. In het eerste deel danst één persoon, vaak met een helm of masker op, terwijl de mensen om hem heen hun normale bezigheden blijven doen, bijvoorbeeld in een kantoor, klaslokaal of andere alledaagse omgeving. Wanneer in het nummer de zin “Do the Harlem Shake” klinkt, volgt een plotselinge overgang, ook wel de “drop” genoemd. Op dat moment verandert de scène abrupt: via een snelle montage is de ruimte ineens gevuld met mensen die verkleed zijn en op een chaotische, ongecoördineerde manier dansen. Within 5 days, over 250,000 videos were uploaded with the same trend, and reaching 1 billion in cumulative views. In this trend, there is one person dancing awkwardly for the first several seconds while everybody in the back act casual. Within 15 seconds, there is a hard DROP followed by an abrupt cut to everyone going berserk in costumes and masks, and flailing randomly. The absurdity and lack of explanation was the magic. No one asked "why”, you just did the dance. The hype lasted almost 3 months into April 2013, as copycats began saturating feeds.
