Obsessive-Compulsive Disorder (OCD) is often misunderstood as being “too clean” or “too organized.” OCD has two very different parts: obsessions and compulsions. Obsessions are thoughts. They are intrusive, unwanted, and often disturbing.They don’t feel like something you chose to think they just appear, again and again, creating anxiety and discomfort. Compulsions are behaviors. They are the actions or mental rituals done to reduce that anxiety like checking, washing, repeating or seeking reassurance. These two are not the same. One happens in the mind, the other is what you do because of it. They are connected but they don’t overlap. The difficult part is the cycle: a thought creates anxiety → a behavior reduces it → but only for a while. And then it starts again. OCD is not about liking things a certain way. It’s about feeling stuck between what your mind tells you and what you feel forced to do to quiet it. The disorder is still misunderstood, especially when it comes to specific themes of OCD. 2. Many individuals have mental compulsions that go unseen to the untrained eye. Individuals with Purely Obsessive OCD or “Pure-O” have intrusive thoughts or images paired with an uncomfortable emotion such as anxiety or guilt. They will then engage in mental compulsions to try and attempt to reduce the emotion, eliminate the intrusion, seek certainty and answers, or try to prevent fear outcomes. The brain found something that worked and it is completely uninterested in your good intentions or your New Year’s resolution. This is why people can understand exactly why they use, genuinely want to stop, try with everything they have, and still find themselves repeating the pattern. In psychoanalysis this connects to repetition compulsion. The behavior doesn’t repeat because someone is weak. It repeats because the emotional system is doing exactly what it was built to do.
