The Cuboid: The Forgotten Bone After an Ankle Sprain... On the outside of your midfoot, there’s a small bone called the cuboid. It doesn’t get much attention, but it plays a key role in keeping your foot stable and supporting the arch when you walk, run, or stand. After an ankle sprain, the cuboid can become stiff or slightly out of place. When that happens, you might feel pain along the outside of your foot, either on the top or underneath. If the cuboid isn’t moving properly, your foot mechanics get thrown off, which can slow down recovery and keep pain hanging around for months. That’s why checking and treating the cuboid is so important. Restoring its movement helps the foot work normally again and can stop those frustrating aches from becoming a long-term issue. 🧠Cuboid syndrome involves pain on the outside portion of your midfoot (i.e. on the pinky toe side of your foot, just in front of your heel bone). This pain is usually worse with standing and waking, and sometimes it can radiate throughout the foot. This condition is often called other names such as cuboid fault syndrome, dropped cuboid, or subluxed cuboid. Cuboid syndrome often occurs after a traumatic event such as an ankle sprain, which can lead to altered stability at the calcaneocuboid joint. It can also occur if you have excessive over-pronation or have prolonged foot positioning in shoes such as high heels. While this condition can be very painful, the altered movement that occurs with cuboid bone is usually pretty subtle. Therefore, cuboid syndrome can be difficult to diagnose with imaging alone and often goes misdiagnosed. Our physical therapists here at BYoung & TheRunningPTs can help determine if your pain is due to cuboid syndrome or another similar diagnosis such as plantar fasciitis. For most of human history, the cuboid adapted constantly to uneven terrain: rocks, roots, slopes, sand. That variability forced the lateral foot to load, unlock, twist, and spring. Fast-forward to today 👟⬇️ Flat shoes. Flat floors. Flat streets. When the ground stops challenging us, the cuboid stops moving. We see lateral foot pain or pain on the outside of the foot pretty often in the clinic. One of the most common causes is the cuboid!🦶 The cuboid is a bone in the midfoot that forms joints with your heel bone, tarsal bones, and 4th and 5th metatarsals. All of these joints are designed to move for optimal foot mechanics during standing, walking, running, and hopping. Sometimes, the cuboid moves just enough to get stuck. It's one of the more painful joint dysfunctions and can be painful when putting weight through the foot, pushing through the toes, running, and jumping. We see it a lot in our running clients. Our treatment approach includes manipulating or mobilizing the cuboid to get it moving better and then addressing the root of the issue--which is often single leg stability and strength for a long term fix. What’s a Cuboid adjustment of the foot? the cuboid is a squarish-shaped bone on the outside edge of the midfoot. It’s attached to the peroneous longus tendon, that long tendon that runs down the outside of your calf and behind the bony bump on your ankle. Unfortunately, impaired muscle function, ankle sprains, overuse, and/or excessive weight can cause this bone to become unstable and subluxate. Cuboid syndrome is a cause of lateral midfoot pain and/or instability arising from a disruption of calcaneocuboid joint integrity due to recurrent or forceful eversion of the cuboid.
