1️⃣ Listening carefully to a patient’s story often reveals the diagnosis before any imaging does. 2️⃣ Endometriosis is not just a gynecologic disease. It frequently involves the bowel, bladder, ureters, nerves, and even the diaphragm. 3️⃣ Negative imaging does not rule out endometriosis. Many cases of endometriosis—especially superficial or early disease—are simply not visible on imaging. 4️⃣ Stage of disease does not equal severity of symptoms. Minimal disease can cause debilitating pain, while stage IV disease can sometimes be surprisingly silent. 5️⃣ Excision surgery is fundamentally different from ablation. Burning the surface of disease is not the same as removing it completely at the root. 6️⃣ Endometriosis is not just a “bad period.” It is a systemic neuroinflammatory disease that can affect multiple organ systems far beyond the uterus. 7️⃣ Many IBS and interstitial cystitis diagnoses are actually undiagnosed endometriosis. Because endometriosis can irritate the bowel and bladder, it often masquerades as gastrointestinal or bladder disease. 8️⃣ The quality of surgery matters more than the number of surgeries. One meticulous, complete excision can be far more effective than multiple incomplete procedures. 9️⃣ Most OB/GYNs are not trained to excise endometriosis — and that’s okay. Interstitial cystitis causes urinary urgency, frequency, bladder pressure, and pelvic pain.  But it’s not just a bladder disease - it’s a visceral pain syndrome with neuropathic changes that can affect the entire pelvis. The Diagnostic Gap: 💥 In women with chronic pelvic pain, studies found that 86% of those with confirmed endometriosis also had IC.  Yet only 8% of IC cases are diagnosed by gynaecologists. Why They’re Missed Together: Both cause near-identical symptoms and without simultaneous investigation of the bladder and pelvis, it can be almost impossible to tell them apart.  IC drives neurogenic inflammation and pelvic floor dysfunction, creating a self-perpetuating pain cycle - meaning that even if endometriosis is treated, the pain can persist. Management Requires Treating BOTH:

🤝Multidisciplinary: Gynaecologists must develop relationships with urogynecologist/urologist 🔬 Consider simultaneous laparoscopy + cystoscopy  🚫 Don’t assume endo is the only cause of pelvic pain 🎯 Bladder: Specific medications are available  - a urologist is best to advise 🎯 Endometriosis: Excision surgery 🎯 Pelvic Floor physical therapy studies show up to 83% moderate-to-marked improvement 🎯 Dietary: Eliminate triggers (may include: acidic foods, caffeine, alcohol, carbonated drinks) When IC was diagnosed early (within 2.5 years of symptom onset), a multimodal treatment approach achieved over 80% improvement in symptoms. Most people think histamine intolerance = hives + sinus congestion. But histamine receptors are in almost every system of your body… so when histamine is elevated (or your body can’t break it down well) it doesn’t always show up as a rash. It can look completely “random.” Here’s what histamine intolerance can look like by body system 👇 🧠 Brain + Nervous System • Migraines (often worse around ovulation) • Anxiety / panic • Racing thoughts • Irritability • Brain fog • Dizziness • Light / sound sensitivity • 1–3am insomnia • “Wired but tired” Histamine is excitatory in the brain → can raise glutamate, activate the stress response, and lower vagal tone. 🌸 Hormones + Cycle • PMS or PMDD • Breast tenderness • Heavy periods • Short cycles • Ovulation pain • Worse symptoms before your period • Perimenopause flares Estrogen can stimulate mast cells… and histamine can stimulate estrogen. ❇️FACT: the definition of interstitial cystitis is “an unpleasant sensation (pain, pressure, discomfort) perceived to be related to the urinary bladder associated with lower urinary tract symptoms of more than six weeks duration in the absence of an infection or other identifiable causes.” ❇️FACT: a patient can be diagnosed with IC/BPS without a cystoscopy ❇️FACT: pelvic floor dysfunction and/or central sensitization are two different subtypes of IC/BPS and can independently cause IC like symptoms by themselves. ✅ REALITY: many patients who come into your clinic for pelvic floor therapy who have been diagnosed with IC/BPS may not have had as thourough of a medical work up as we would like and its our job to make sure our patients get the care they deserve! ▶️ So here are three things that are high on my radar when someone comes in for pelvic floor physical therapy with IC/BPS: 1️⃣A recurrent infection that has not been properly worked up. This can be a true culture positive UTI or something like a recurrent BV infection, where the pateint and/or the partner was not adequately treated 2️⃣ Endometriosis (or adenomyosis). As we know, Endo is significantly underdiagnosed. Take into consideration a patient’s menstrual cycle history, current tracking and putting pieces together with the onset of bladder pain and potential endometriosis signs and symptoms. The most notable would be severely painful and debilitating menstrual cycles, severe bloating, and painful bowel movements. 3️⃣ Any sign of destrogenized local tissues at the vulvovaginal and urethral region. This can stem from a variety of causes: GSM, GSL, perimenopause or prolonged use of oral contraceptives. Each of these areas would require different medical treatments and potential referrals to our medical colleagues. BUT concurrent with our pelvic floor physical therapy interventions. 💁🏻‍♀️ Questions? What is interstitial cystitis? 🩵 You may have never heard of this and that’s ok, or you may have! Actress @lilireinhart talked about this when she was diagnosed and so today I am here to tell you that I now I have it too. I have interstitial cystitis. This is also referred to as painful bladder syndrome and it’s a chronic illness that does exactly that, causes severe pain. I knew I already had severe pain from endometriosis and that Endo had been found on and inside my colon and bladder so that unfortunately caused a lot of pain and problems. Abdominal or severe abdominal pain is nothing new to me. During this last laparoscopic Endo surgery I also had a colonoscopy and cystoscopy and while they had the camera inside my bladder they could see the lining was different and diagnose me. Since, I have seen a urologist and started taking medicine to help me sleep through the nights better (I have woken up in pain for years) and I’ve been told I have to eat a more healthy bland diet to keep the flare ups more manageable. For years I’ve had pain. More than a decade of chronic abdominal pain. Fifteen plus years ago I knew something was off and eventually ten years ago I got diagnosed with Endo. I knew I had Endo on/in (whatever) my bladder in the past few years because I had extreme pain after drinking water and when my abdomen would expand and contract. They found it during my third surgery. This time, they found this. I’m not upset. I’m not angry or wondering why. I’m grateful to have an incredible doctor, that listens to me, believes me and knows what to look for based off his experience. I’m grateful to live in a city with incredible doctors and nurses that know what they’re doing and I’m grateful to have more answers. Still haven’t figured out why I get sick all the time and we are still learning more and more about Endo every year. Doctors don’t know if my bladder Endo caused this or if it was there, but hopefully one day I’ll have more answers. Praying that one day there is a cure for all of these things and hoping, in the meantime, I can help someone else going through a similar journey. 🙏🩵💛 #interstitialcystitis #endometriosis #endo
