‼️We have a different but very commonly ordered genetic test to feature this week: chromosomal microarray! Unlike whole genome sequencing and panel testing featured in our other videos, microarray testing doesn’t look at the spelling of individual genes, but looks at segments of chromosomes that contain multiple genes. 🧬 One way we sometimes describe this to families is thinking of our genome as a library. 📚 Instead of reading all the books for errors, we are looking at the shelves and whether there are extra books (duplications) or missing books (deletions) on each shelf. 📖 This test is super useful, especially in patients with multiple systems affected, such as babies in the NICU with multiple congenital anomalies. It also has the advantage of being much cheaper 💵than more comprehensive testing like WGS, but at the expense of less information. You can use this test to diagnose conditions like 22q deletion syndrome, cri du chat syndrome, and wolf-hirschhorn syndrome. 👩‍⚕️👨‍⚕️ What are some other conditions that you have seen or diagnosed with a microarray? 🤔👇

After a normal first trimester screening, our 20-week anatomy scan with Marsaili revealed several abnormalities. We began a cascade of testing: 🔬 Amniocentesis (a procedure that takes a small sample of amniotic fluid) was sent for karyotype and microarray testing—both looking for chromosomal issues. 🧬 Results came back normal, so we pursued whole exome sequencing (a deep dive into the protein-coding regions of DNA) using samples from me, Josh, and Marsaili. 🧠 I also had a fetal MRI, where we first heard the word lissencephaly—a rare brain malformation meaning “smooth brain,” which can lead to many complications including developmental delay. Despite all the testing, we still had no clear answers. Every complication seemed to point to something genetic—but what? We wouldn’t know how it would affect her life until she was born. All we could do was wait and pray and keep fighting. At 34 weeks, I was admitted for severe polyhydramnios (excess amniotic fluid), which was worsening quickly. Today we received results from Josiahs chromosomal microarray testing. And it came back PERFECT! This was done to see if there are any chromosomal abnormalities that would explain his low tone and struggle reaching some milestones such as standing and so forth. It’s really reassuring and good to know that no abnormalities were found. I often still wonder why Josiahs hypotonia is the way it is but the more and more we dig into things the more I begin to understand that his development and timeline looks completely different from anyone else’s. While I knew this all along I still find the need to often reassure and remind myself of this. We continue to work daily on all the things. Josiah is now able to sit with minimal support, he is still struggling to bare weight on his feet but I think with improved core strength this will follow. and that’s definitely a department we are working on all the time. Sometimes when I get discouraged I start to look at Josiahs big picture progress rather than the now and then I am quickly reminded how far our boy has come ! . . . . . . #development #milestones #baby #nicubaby
