LEMany new moms are told, “Your ejection fraction is low,” without truly understanding what that means, especially when Peripartum Cardiomyopathy (PPCM) is involved.
Ejection Fraction (EF) is the percentage of blood the left ventricle pumps out with each heartbeat. A healthy heart doesn’t empty completely, but it should pump effectively.
• Normal EF: 55–70%
• Borderline/Mildly Reduced: 41–54%
• Moderately Reduced: 30–40%
• Severely Reduced: Below 30%
In PPCM, EF is often below 45%, and sometimes much lower, which means the heart is struggling to meet the body’s needs, especially during postpartum recovery.
PPCM is not diagnosed based on one symptom alone. Doctors look at a combination of factors:
• Symptoms of heart failure during late pregnancy or up to 5 months postpartum
(shortness of breath, swelling, extreme fatigue, chest discomfort, rapid heart rate, inability to lie flat)
• Echocardiogram (heart ultrasound) showing reduced ejection fraction and weakened heart muscle
• No prior history of heart disease before pregnancy
• Blood tests (like BNP/NT-proBNP) that indicate heart strain
• Sometimes additional imaging or monitoring to rule out other causes
⚠️ PPCM is often misdiagnosed as anxiety, normal postpartum changes, or exhaustion, this is why education and early screening matter.

DREjection Fraction estimation at the bedside using EPSS.
EPSS (E-point septal separation) is the distance between the anterior mitral valve leaflet (E-point) and the interventricular septum on M-mode echo.
🔍 How it works
• Obtain parasternal long-axis (PLAX) view
• Place M-mode cursor through mitral valve tips
• Measure the minimum distance between E-point and septum
📊 Interpretation
• EPSS < 7 mm → Normal LV systolic function
• EPSS > 7 mm → Reduced EF
• Higher EPSS = Lower EF
As a rough guide:
👉 EF ≈ inversely proportional to EPSS
🧠 Clinical Use
• Rapid assessment in shock, dyspnea, ICU/ER
• Helpful when formal echo is not immediately available
• Best used as a screening tool, not a replacement for full echo
⚠️ Limitations
• Not reliable in mitral valve disease
• Altered in LV hypertrophy or regional wall motion abnormalities
• Always correlate with clinical context
