❤️‍🩹Let’s Talk about HEART BLOCKS There are several types, and they go in order from 1st degree to 3rd degree (or complete!) First Degree: In 1st degree heart block, there is a noted prolongation of the PR interval. This is the primary difference between first degree heart block and normal sinus rhythm Second Degree (Wenckebach or Mobitz Type I): The P waves will gradually reach further and further away from the QRS complex, and eventually DROP entirely Second Degree (Mobitz Type II): The PR interval here remains the same, but the QRS complex will eventually drop, leading to a missed beat. These patients often need to be paced! Third Degree (Complete): The most severe type of heart block is a complete heart block. Here, P waves are entirely unmatched with QRS complexes. Think of this as a miscommunication altogether in cardiac activity! Pacing is also required here to realign the communication between the atria and ventricles. Each small square represents 0.04 seconds (40 milliseconds). 4. **Calculate the duration**: Multiply the number of small squares by 0.04 seconds to get the duration of the PR interval. A normal PR interval ranges from 0.12 to 0.20 seconds (120 to 200 milliseconds). Today’s feature! @nurselambrn Save 📚Share 🎙️ Bookmark ☑️ for your reference One thing as a nurse I did not care for was interpreting EKG strips. But there was no way to get around it, it is a part of my daily nursing task. I struggled with understanding the difference between 1st-degree, 2nd-degree type 1, and type 2 heart blocks. I just got tired of relearning this information each time I took on a new travel nurse assignment. So I just started using a very simple approach to the information. EKG interpretation is very systematic, and all you have to do is take this same approach with every 6-second strip. When interpreting a 6-second strip, use the following steps: 1. Determine the rate- count the number of R waves and multiply by 10. This will give you a rough estimate of the rate. The normal rate is between 60-100 BPM. 2. Determine if the rhythm is regular or irregular- take a piece of paper and place two marks on two consecutive R waves (R-to-R interval). Use this paper to measure the R-to-R interval for the remainder of the strip. Is it regular or irregular? 3. Find the P Waves- Not all rhythms have P waves. Determine if they are present. 4. Measure the PR interval- for this, remember that each small box is 0.04 seconds. Count the boxes to determine the interval. The normal PR interval is between 0.12 and 0.20 seconds. 5. Determine the QRS interval- Use the same method to count the QRS interval as done in step four. The normal QRS interval is no more than 0.12 seconds. Now you can name the rhythm. Reminder, take a look at the T waves, as abnormal findings such as ST-segment elevation greater than 1 mm can be a serious finding.
