Is not known to increase stroke risk and at this time, there are no guidelines to give medications like Aspirin, or to close a PFO PROPHYLACTICALLY to prevent stroke. However, we do know patients with cryptogenic stroke are more likely to have a PFO. We also see an association between migraine with aura and PFO. We still need more studies to understand when it is best to do a procedure to close the PFO. If you have had a stroke and you are younger than 60 (although a recent study says close at any age), and, you also have a PFO, AND, we can’t find another reason for your stroke, then the PFO should be closed. Symptoms that may be associated with PFO include: TIA/stroke, dizziness, palpitations, migraine, chest tightness. Most people with PFO never have symptoms. I wonder, however, if patients with pelvic venous disease are more likely to have symptomatic PFOs……. Please see next post for papers. 🩺 This is NOT medical advice. Usually the cause of strokes is apparent and often related to high blood pressure or atherosclerosis; but in younger patients who present with mini strokes (TIA) or stroke, they are diagnosed often times with “cryptogenic stroke” of unknown cause. It is really important in these patients to screen for a patent foramen ovale (PFO), a hole in the heart that usually closes shortly after birth, but in 20-25% of the population this remains open. Often this does not present any problems later in life, but in a subset of patients it can be associated with stroke. A stroke can happen when a blood clot in the veins travels to the heart, bypasses the lungs, and instead crosses through the hole (PFO) and travels to the brain. PFO screening is via a Transthoracic echo using a bubble study, and confirmed by a transesophageal echo. If a patient has had a stroke and has a PFO as well, they may be a candidate for PFO closure to prevent future strokes. This 39yo man had a hemorrhagic stroke and ultimately was found to have a PFO, deemed the cause of his stroke. We were able to close his PFO in the cath lab with a 30mm Gore cardioform PFO closure device! Most never know it’s there… but for some, it can be a silent culprit. 🫀Indications for Closure: ✅ Cryptogenic stroke (ischemic stroke without another cause) in patients ≤ 60 years with a moderate-to-large shunt or atrial septal aneurysm. ✅ Recurrent embolic events despite optimal medical therapy. ✅ Platypnea–orthodeoxia syndrome — oxygen drops on standing, relieved when lying down.
