Heard about the Trail's sign? 🫁 One of the most widely used method to check the centralization of the trachea. Follow these steps to check for any deviation in the position of the trachea which may tell you presence of any unilateral lung problem. -Various conditions cause the trachea to be either pulled to the same side (eg. Lung Collapse ) or pushed to the opposite side (eg. Pleural effusion) of the problematic lung. -Few conditions may lead to a false positive Trail's sign (Trachea is shifted but not due to a lung condition) such as scoliosis, clavicle fracture, torticollis etc. -While conditions like bilateral lung conditions or two conditions that may oppose the tracheal position ( eg. presence of plural effusion and consolidation on the same side) may give a false negative result. How to Spot a Pneumothorax on Chest X-ray When you’re faced with a chest film and suspect a pneumothorax, here’s what to look for: 1️⃣ Absent Lung Markings – Beyond the visceral pleural line, there are no vascular markings. 🔴Dont confuse it with the tracheal tug(Oliver sign),which is seen in Aortic arch aneurysm .It's downward movement of the trachea during systole. 🔴How to elicit: While the patient stands with their chin extended, standing behind the patient gently grasp the cricoid cartilage and apply upward pressure 🔴The aortic arch overrides the left main bronchus, so if an aneurysm is present, a downward tug of the trachea may be felt. 🔴Falsely positive can be seen in mediastinal tumor attached to aortic arch.Falsely negative can be seen in non-pulsatile aortic arch aneurysm. Tracheal deviation with absent breath sounds and sats dropping may suggest respiratory compromise. 2. 700 mL bright red blood in 30 min can suggest active hemorrhagic shock. 3. CHEST X-RAY REVIEW AREAS IN 90 SECONDS I can’t stress the importance enough of using a system to go through a chest X-Ray - I learnt this the hard way but without going through the same rigorous search pattern on every film you’re at risk of missing a really important finding. I usually scan the film first looking for and evaluating any major abnormality then go through the following: 1. APICES: look for a pneumothorax and pancoast tumour - I do this first as these can be subtle 2. RIBS: zoom up and follow all the anterior ribs then follow all of the posterior and lateral ribs 3. CLAVICLE AND SCAPULA: whilst you’re zoomed up assess the clavicle and scapulae for fracture or bone lesion 4. VERTEBRAL BODIES: if you can see them assess the vertebral bodies for loss of height and density 5. MEDIASTINUM/HILAR REGIONS: use the mnemonic PATACAKE. Paratracheal region, Azygous contour, Tracheal deviation, Aortic contour and AP window, Carina splaying, Azygo-oesophageal recess, K-shaped hilar regions, Everything else (behind the heart, heart contour, pneumomediastinum) 6. PINBALL: like a pinball machine we’ve gone down the mediastinum then go down and look below the diaphragms for free gas and lung lesions before coming back round and following the lung peripheries for pleural or chest wall lesions 7. INVERT: inverting the film gives your eyes another chance to assess the film 8. Patient Details: Age: 30 years Gender: Female Clinical History: Painless left neck mass for 2 years, with recent increase in size and difficulty in swallowing. Findings: A lobulated, non-calcified soft tissue density lesion is seen in the left side of the neck. The lesion is causing mass effect with displacement of the trachea towards the right side. No obvious calcification or air foci within the lesion. Surrounding structures are displaced but not clearly invaded (on this imaging). Impression: Large left-sided neck mass causing tracheal deviation. Features are suggestive of a thyroid origin mass (likely multinodular goiter / thyroid neoplasm). Advice: Ultrasound neck for further characterization. Thyroid function tests (TFTs). FNAC (Fine Needle Aspiration Cytology) for definitive diagnosis. Consider CT/MRI neck for extent and surgical planning.

Pacientes com mais sinais de congestão (incluindo distensão venosa jugular, edema, estertores e terceira bulha) apresentam maior risco de morte cardiovascular ou hospitalização por insuficiência cardíaca, independentemente dos sintomas, peptídeos natriuréticos e escores de risco validados.
