Investigation of Choice for some important Respiratory Diseases. Interstitial Lung Disease — the investigation of choice is HRCT. Bronchiectasis — again, the best investigation is HRCT. It shows the classic tram-track and signet ring appearance. Pulmonary Embolism — the investigation of choice is CT Pulmonary Angiography. If that’s not available, use a Ventilation-Perfusion Scan. Pulmonary Calcifications —CT scan. Solitary Pulmonary Nodule — the first investigation is CT, and if needed, PET scan is used to assess metabolic activity. Lung Malignancy — CT scan is the primary imaging choice. Pulmonary Embolism (PE) 🫁- Pulmonary Embolism is a sudden blockage of one or more pulmonary arteries in the lungs, usually due to a blood clot (thrombus) that has travelled from the deep veins of the legs – a condition known as Deep Vein Thrombosis (DVT). This blockage reduces blood flow to lung tissue, causing sudden shortness of breath, sharp chest pain, and sometimes coughing up blood (hemoptysis). Severe cases may lead to shock or cardiac arrest. --- Key Exam Points (with Full Forms) Most common source: DVT – Deep Vein Thrombosis Classic Triad: Dyspnea (shortness of breath) + Chest pain + Hemoptysis (blood in sputum) Risk Factors: Surgery, long immobilization, pregnancy, oral contraceptive pills, cancer, smoking, obesity. Diagnosis (with Full Forms): D-Dimer Test – Detects clot breakdown products CTPA – Computed Tomography Pulmonary Angiography (Gold standard) V/Q Scan – Ventilation/Perfusion Scan Doppler Ultrasound for DVT detection Treatment (with Full Forms): LMWH – Low Molecular Weight Heparin UFH – Unfractionated Heparin DOACs – Direct Oral Anticoagulants (Examples: Apixaban, Rivaroxaban, Edoxaban, Dabigatran) Thrombolysis in massive PE (e.g., Alteplase) IVC Filter – Inferior Vena Cava Filter in selected cases ....... They aid in improving vision, ergonomics such as poor posture, stamina due to decreased visual strain, and overall patient care! • Mine are from SurgiTel - 2.5 magnification - rose gold and run around $1500 which is pretty typical for loupes. If you need them for your position try to get your employer to help cover the cost! On chest radiography, the Westermark sign (focal oligemia) and Hampton hump (a peripheral wedge-shaped opacity) are classic findings, but they are uncommon. The ECG is abnormal in 80–90% of patients, although the changes are usually nonspecific and not diagnostic.
