Two months on an elimination diet and there’s no improvement? Eek! There’s so much misinformation out there about how long it takes non-IgE allergy symptoms to resolve. I’ve had so many clients told it takes weeks to months. And while that may be true to see total resolution, you should be seeing some noticeable improvement within 7-10 days. If you’re not, there are a couple of reasons why: 1. The symptoms aren’t caused by food allergies. 2. You ate hidden sources/had an accidental exposure. 3. The food(s) you cut are not the ones your baby is allergic to! If this sounds familiar - don’t feel bad. You can’t know what you don’t know. But now that you know, please reach out! 💡 New research on FPIAP might change how long some families wait before reintroduction! A 2024 study in Pediatric Allergy and Immunology followed 61 infants diagnosed with food protein induced allergic proctocolitis (FPIAP) and tracked both their symptom resolution and gut microbiome changes. 🦠 AP affects the lower GI tract and can cause mucus in stool, blood in stool, diarrhea, constipation, and related secondary symptoms like inconsolability, weight gain concerns, and sleep disturbances. 🫩 These were babies with AP alone, not AP plus other non-IgE mediated allergy symptoms like eczema, vomiting, reflux, or other inflammation. 🤕 So more research is needed around combo-symptom babies and whether they should also shift to 3 month elimination or stick to the current 6 month recommendation. Researchers found that 96.5% of the infants tolerated milk again after just 3 months of elimination. 🥳 On average, tolerance occurred at 6.3 months. In the gut, they saw a shift from Enterobacteriaceae-dominant bacteria early in the allergy to higher levels of Bacteroides and Bifidobacteria as tolerance developed. 🥛 This shift happened naturally, without probiotic supplementation in the study. 💊 For many families, this could mean building a reintroduction action plan earlier than expected. 🚨 Blood in stool ≠ “dairy allergy.” 🚩There are many causes for blood in stool, including fissures** (below for this one!), infection, food reactions & more. 🌟Here’s what the science actually says ⬇️ 1️⃣ Cow’s milk and sometimes soy ARE the most common triggers for Food Protein–Induced Allergic Proctocolitis (aka FPIAP). ⚠️BUT FPIAP is frequently MULTI-food sensitive, not a single allergen. 👉 Many cases react to multiple proteins — not just dairy. (Mennini 2020; Nowak-Wegrzyn 2017) 2️⃣ Babies can be exposed to food proteins through breastmilk or formula. ✅This is why elimination is done at the parent diet level (if nursing) OR formula level. ***FOOD PROTEIN-INDUCED ALLERGIC PROCTOCOLITIS (FPIAP)*** . . ~WHAT IS IT: AKA milk protein sensitivity. Common problem in young infants. Inflammation of the distal colon in response to one or more food proteins through a mechanism that does not involve immunoglobulin E (IgE). Cow’s milk and soy protein are the most common triggers. Can occur in formula-fed and breast-fed infants. . . ~SYMPTOMS: Usually begin in the first few weeks or months of life. Frequent visible blood in normal or loose stools, loose stools with mucus, with or without gross or occult blood, constipation, usually with occult blood (uncommon presentation), irritability. . . ~Diagnosis: Almost always a clinical diagnosis, based upon typical presenting symptoms that resolve once offending food is taken out of diet. . . ~Management: Breast-fed infants: Exclusively breastfed infants — Continued breastfeeding should be encouraged if the mother is willing to completely eliminate the suspected food from her diet. Cow’s milk and soy should be eliminated first. All dairy products should be completely eliminated from the mother’s diet, including butter. All other mammalian milks (eg, from goats, sheep, or camels) should also be eliminated because of cross-reactivity. For infants with severe symptoms, it may be helpful to accelerate the dietary elimination by using an amino acid-based formula rather than breast milk for three to five days, while the mother pumps and discards her breast milk (to wash out the antigens from the mother’s body) to maintain lactation. Cow’s milk proteins also may be listed as “casein” or “whey” on some processed food labels. Formula-fed infants: Cow’s milk formulas should be replaced with an extensively hydrolyzed formula like Nutramigen or Alimentum. Changing to a soy-based formula is not recommended. Approximately 5 percent of infants with cow’s milk- or soy-induced proctocolitis do not respond to feeding with an extensively hydrolyzed cow’s milk formula and continue to bleed. Part 1 - I was asked in a question box about our mealtimes and tried to include as much detail as I could in this reel. I have been sharing more on our FPIAP allergies and gut bleeding journey in my story past few months. He has made wonderful progress and today we have been given all clear to start him on dairy ladder through solids after phase of symptom free breast milk dairy exposure. CMPA has been remaining protein trigger after we have progressed with others past few months. In addition to FPIAP we are also managing a range of IGE allergies. After nearly a year of being on an elimination diet including dairy, soy and egg, I have new found understanding and insights as to what some families lived experiences are. There is something that pulls at your heartstrings being excluded from meals (even after checking, offering to bring own food and being reassured it would be fine) and I’m a 34 year old woman. This often made me reflect on so many little kids who face this experience daily - at schools, special gatherings, birthday parties. Dismissive comments like “ugh one lick won’t hurt” or “cmon it can’t be that bad, live a little”. His gut bleeding cries broke my mama heart and is something I hope to never experience again. We are nervously excited for this next phase of his allergy journey. I will need to make sure I am well regulated at meal times where we will be introducing dairy to him over next few months. We will continue to monitor and test in this next phase and see how we progress along the dairy ladder 🤞 Fellow allergy mamas have you experienced dismissive comments or difficult situations as I mentioned above? Аллергический проктоколит(FPIAP)-состояние, основным симптомом которого является появление прожилок крови в стуле у в остальном здорового ребенка. Прожилки крови в стуле появляются в течении 48, реже 72 часов после употребления продуктов мамой(при ГВ) или ребенком. Чаще всего виноваты молочные продукты, соя, пшеница, яйцо. Считается, что другие продукты не могут проникнуть в молоко, однако в практике аллергологов встречались такие продукты как кукуруза, курица, греча. Этот момент остаётся дискутабельным. ❕Важно: анализы, тесты не информативны. Необходимо провести диагностическую диету-исключить продукты, прожилки крови должны исчезнуть в течении 72 часов. В некоторых случаях может понадобиться двухнедельная диета. Определяется врачом индивидуально. И логичный вопрос, это состояние пройдёт? Действительно, большинство детей «перерастут» такой вид аллергии. Обязательно обратитесь к врачу-аллергологу для определения состояния ребенка. Прожилки крови встречаются также при анальных трещинах, инвагинациях, кишечных инфекциях. @pediatr__belova - здесь все о педиатрии и аллергии

Именно в этом случае детский аллерголог и гастроэнтеролог вот так за ручки пойдут обсуждать пациента! 👯‍♀️ И одна из самых распространенных причин примеси крови в стуле - это аллергия на белок коровьего молока или FPIAP (Food protein-induced allergic proctocolitis - проктоколит, обусловленный аллергией на пищевой белок). Начинается с первых недель жизни и длится по-разному, чаще проходит в возрасте 12 месяцев. Наиболее частые триггеры - белок коровьего молока и соевый белок 🥛🫘 Еще одно расстройство, связанное с реакцией на БКМ - это FPIES (Food protein-induces enteropathy), при котором поражена бОльшая часть ЖКТ, а симптомы более выражены. Проявления FPIAP: - видимая кровь в стуле + диарея - реже кровь в стуле + запор - обильно примесь слизи в стуле + диарея - возможны хорошие прибавки в весе и активный, в остальном здоровый младенец. И все же данные симптомы не специфичны и могут встречаться при различных состояниях, диагноз и дальнейшие рекомендации устанавливаются только врачом❗️ Диагностика : Как правило, врачу достаточно осмотреть младенца и собрать анамнез для установки диагноза. Обязательно в осмотр входит - оценка динамики роста/веса📈 - осмотр ануса на предмет трещин🍑 - оценка стула (желательно разных порций)💩 Лечение: - младенцы находящиеся полностью на ГВ - сохранение грудного вскармливания при условии соблюдения мамой диеты 🤱🏻 - младенцы, получающие смесь переводятся на смесь с расщепленным белком 🍼 Еще больше полезного читай в блоге ☘️ Детский аллерголог @doctor_sharova.am Детский гастро @doctorannna
