AYA thoughtful measles question came in: “If I’m fully vaccinated but got measles, could I transmit it to my infant?”
A few key concepts:
1️⃣ R₀ (R-naught)
R₀ is how many people one infected person would spread a virus to in a fully susceptible population.
For measles, R₀ = 12–18. Extremely contagious.
But that’s not the right number to use when we’re talking about a vaccinated person in a vaccinated or partially vaccinated community.
2️⃣ Secondary vaccination failure (SVF)
Rarely, someone who previously responded to the vaccine can develop measles years later due to waning immunity. This is called secondary vaccination failure.
In these cases:
• Illness is often milder (“modified measles”)
• Viral load tends to be lower (higher PCR Ct values)
• Immune memory is present but incomplete
That immune memory appears to reduce spread.
3️⃣ What does transmission look like?
A 2023 systematic review (14 studies, 109 SVF cases) found:
• Effective reproduction number (Re) ≈ 0.06–0.21
• 23 total secondary cases from 109 SVF cases
• Among >3,000 exposed persons, only 5 infections occurred
• Secondary attack rates ranged 0–6.25% (among susceptible contacts)
Translation:
Most vaccinated breakthrough cases transmitted to NOBODY. A small number transmitted to one or more people, usually with prolonged close exposure (household, healthcare, confined settings).
That is very different from classic measles in an unvaccinated population. Vaccination lowers viral load and pushes the effective reproduction number down.
Transmission risk is low but not zero, which is why public health follow-up still matters — especially around infants and immunocompromised contacts.

In all seriousness for a second, it is OKAY to have questions and to feel some level of anxiety as a parent when your baby is due for vaccines. I even understand that parents of children who are diagnosed with a variety of conditions around the time they receive vaccines in early childhood would want to look for answers and be suspicious of vaccines. KOThe Snohomish County Health Department has confirmed three additional measles cases since declaring an outbreak two weeks ago, bringing the total number of cases in the county to six.
The latest case was confirmed on Tuesday in an unvaccinated child.
Health officials said the child was infectious while attending an afternoon service at Slavic Christian Church Awakening in Mukilteo on Sunday, Jan. 18.
Anyone who attended the church between 2 p.m. and 6 p.m. that day may have been exposed to the virus, the health department said.
