Progression of Macular Hole from Stage 1 to stage 4 A macular hole is a full-thickness defect of the neurosensory retina at the fovea, most commonly caused by vitreomacular traction during posterior vitreous detachment. This traction leads to progressive disruption of the foveal architecture and photoreceptors, resulting in painless central vision loss, metamorphopsia, and central scotoma. It is typically age-related but may also occur following trauma or in highly myopic eyes. Optical Coherence Tomography (OCT) is the gold standard for diagnosis, clearly demonstrating the structural changes and guiding staging and management. Macular holes are classified into four stages - . Stage 1 (impending hole) shows loss of the foveal depression with foveal detachment - Stage 2 is the formation of a small full-thickness hole (<400 µm), often with persistent vitreofoveal adhesion and early cystic changes. - Stage 3 represents progression to a larger hole (≥400 µm) with surrounding intraretinal cysts and a cuff of subretinal fluid, while the posterior hyaloid is partially detached. Stage 4 is similar to Stage 3 but with a complete posterior vitreous detachment, often evidenced clinically by a Weiss ring. On OCT, early stages show vitreofoveal traction, loss of foveal contour, and pseudocyst formation, while advanced stages demonstrate a full-thickness retinal defect with well-defined edges, intraretinal cystic spaces, and subretinal fluid. The status of the vitreomacular interface (attached vs detached) is key in staging. OCT not only confirms diagnosis but also determines hole size and prognosis, which are critical for management decisions, especially regarding surgical intervention. #medicallife #eyes #eyecare #oct

Ele funciona como uma espécie de “tomografia da retina”, permitindo identificar líquido, inflamação, atrofia e outras alterações importantes de forma extremamente precisa. Mas além do OCT, também existem outros exames que podem ajudar na avaliação da DMRI, como a retinografia, a angiografia e até exames mais modernos que analisam os vasos da retina em detalhes. Cada um deles tem um papel importante dependendo do estágio e da forma da doença. O mais importante é entender que o diagnóstico precoce pode fazer toda a diferença na preservação da visão.
