It is immediately placed in formalin to preserve it. ⸻ 2️⃣ Gross Examination (Macroscopic Exam) In the pathology lab: • Pathologist examines the tissue with naked eye. • Notes size, color, texture. • Measures margins if it’s a surgery specimen. • Selects representative areas. Small pieces are trimmed and placed into labeled cassettes. ⸻ 3️⃣ Tissue Processing The cassette goes into an automated processor: • Fixation – Formalin preserves structure • Dehydration – Water removed using alcohol • Clearing – Alcohol replaced with xylene • Infiltration – Liquid paraffin wax enters tissue This prepares it for slicing. ⸻ 4️⃣ Paraffin Block Preparation The processed tissue is embedded in molten wax. It cools and becomes a solid paraffin block. This block is the permanent record. ⸻ 5️⃣ Microtomy (Ultra-Thin Slicing) A machine called a microtome cuts extremely thin slices (about 3–5 microns thick). These slices are placed on glass slides. ⸻ 6️⃣ Staining (Most Common: H&E) Slides are stained with: • Hematoxylin – stains nuclei blue • Eosin – stains cytoplasm pink Now the cells are visible under microscope. ⸻ 7️⃣ Microscopic Examination Pathologist examines: • Cell shape • Nuclear changes • Invasion • Margins • Grade • Type of tumor Diagnosis is made here. ⸻ 8️⃣ Special Tests (If Needed) If cancer is suspected: • Immunohistochemistry (IHC) • Molecular testing • Genetic markers These help in targeted therapy decisions. ⸻ 9️⃣ Storage in Tissue / Biopsy Bank • Paraffin blocks are stored for years (often 10+ years). • Slides are archived. • They can be reused for future testing if required. This is crucial for second opinions or advanced testing later.
