DRNessa parte 2 sobre o que vi no USCAP 2026, trago um dos pontos centrais: o refinamento dos critérios que realmente impactam o risco de recorrência no câncer de tireoide.
Alguns conceitos importantes:
Extensão extratireoidiana macroscópica:
É a que realmente conta. Aquela visível intraoperatória ou com invasão evidente de estruturas adjacentes. A extensão microscópica tem muito menos impacto prognóstico.
Linfonodos:
Não basta dizer se há metástase. É necessário detalhar:
- tamanho do foco metastático
- comprometimento por compartimento (central vs. lateral)
- presença de extensão extranodal
Margem cirúrgica:
Especial atenção para a margem posterior, que está associada a pior prognóstico quando comprometida.
Invasão vascular:
Nas neoplasias encapsuladas, é um dos fatores mais relevantes, sendo:
- até 3 vasos → melhor prognóstico
- ≥4 vasos → maior risco de recorrência
Subtipo histológico:
Variantes mais agressivas (células altas, hobnail, colunar) podem estar associadas a pior prognóstico, especialmente quando combinadas com:
- alto índice mitótico
- presença de necrose
Esses critérios, analisados em conjunto, são fundamentais para a estratificação de risco e definição de conduta.
Esse foi mais um dos highlights discutidos no USCAP 2026.
