While their appearance can be alarming, they are most frequently the result of simple mechanical trauma to the nail bed. Because the capillaries in the nail bed are oriented in longitudinal rows following the grooves of the nail, any minor impact such as stubbing a toe, hitting a finger with a hammer, or even aggressive manicuring can rupture these vessels. In cases of trauma, the hemorrhage is usually confined to one or two nails and is typically located in the distal or outer half of the nail. As the nail grows, the small pocket of trapped blood moves outward with the nail plate until it eventually disappears. Despite their common association with injury, splinter hemorrhages are also a classic albeit rare clinical sign of infective endocarditis. In this serious condition, bacteria or other pathogens infect the inner lining of the heart and its valves, forming vegetations or clumps of infection. These clumps can break apart and send tiny septic emboli, which are essentially infected micro-clots, into the bloodstream. When these clots lodge in the small capillaries of the nail bed, they cause the localized bleeding that manifests as a splinter. Unlike traumatic splinters, those caused by endocarditis are more likely to appear on multiple nails, occur closer to the base or proximal part of the nail, and are often accompanied by systemic symptoms such as fever, fatigue, or a new heart murmur. Because they can signal a life-threatening infection, any unexplained splinter hemorrhages that appear across several fingers without a known injury warrant a professional medical evaluation. #nails #interesting #health
