Every time something goes wrong, someone creates a new checklist. A new form. A new “documentation requirement.” Fall? Add a reassessment checkbox. CLABSI? Add a line maintenance checklist. Med error? Add a double-verification step to the already 14-step process. Nobody asks: is this checklist actually going to prevent the next event? Or is it just going to take 5 more minutes of a nurse’s already impossible shift? Checklist fatigue is real. When nurses have 47 checkboxes to click, they start clicking without thinking. It becomes compliance, not safety. And when the checklist fails to prevent the next event, leadership says “the nurse didn’t complete it properly.” Real quality improvement changes the PROCESS. It removes steps that don’t add value. It redesigns workflows so the safe choice is the easy choice. It builds safety into the system instead of stacking it onto the nurse. As a CNS, we evaluate whether interventions are actually improving outcomes or just creating documentation burden.
