This is known as pitting edema, and it’s more than just swelling. It can be an important clinical clue that the body is retaining fluid. When the heart isn’t pumping efficiently—especially in congestive heart failure (CHF)—fluid can back up into the tissues. The legs and ankles are common places for this to show up because they’re dependent areas where gravity pulls fluid down. Why it matters: • Persistent indentation often signals elevated venous pressure and fluid overload. • This may reflect reduced forward flow from the heart and increased pressure in the venous system. • It typically appears alongside other signs such as weight gain, shortness of breath, or visible vein distension. For clinicians, always pair this finding with a full cardiovascular assessment: checking bilateral pulses, evaluating skin temperature and color, and correlating with blood pressure and overall volume status. Procedure: The patient stands with feet together, arms at sides (or crossed), and eyes open to establish a baseline for 30 seconds. The patient closes their eyes for another 30–60 seconds. A clinician should stand nearby to ensure safety. Positive Result (Romberg's Sign): The patient is stable with eyes open but falls or sways significantly upon closing their eyes.
