T cells are developed from bone marrow (BM)-derived thymocyte progenitors in the thymus, and broadly grouped into CD4+ and CD8+ ฮฑฮฒ T cells in addition to rear populations of ฮณฮด T cells and NKT cells. ฮฑฮฒ T cells recognize antigens that are presented by major histocompatibility complex (MHC) molecules on antigen-presenting cells (APCs). ๐Ÿ”ฌ Why do CAR-T cells struggle in solid tumors? CAR-T therapy has shown remarkable success in blood cancers, but solid tumors remain a major challenge. One of the key barriers is the tumor microenvironment, which actively suppresses immune responses. A major player in this suppression is tumor-associated macrophages (TAMs). Macrophages normally play essential roles in our body - clearing pathogens, removing dead cells, and coordinating immune responses. But tumors have learned how to reprogram these cells to work against us. Inside tumors, macrophages often adopt a pro-tumor phenotype that supports tumor growth, promotes tissue remodeling and angiogenesis, and suppresses anti-tumor immune responses, including CAR-T cell activity. One important inhibitory receptor involved in this process is LAIR1. ๐ŸŽฅ What youโ€™re seeing in this video This experiment uses a live 3D tumor model with real-time imaging. All immune cells were derived from the same human PBMC donor and fluorescently labeled: ๐Ÿ”ด CAR-T cells ๐ŸŸข tumor-associated macrophages ๐Ÿ”ต CD70โบ glioblastoma tumor cells โš ๏ธ What happens in the control condition When macrophages are present, CAR-T cells become suppressed. Their migration is impaired, they interact less with tumor cells, and tumor killing is strongly reduced (first part of the reel!) This demonstrates how TAMs can act as a physical and functional barrier to T-cell activity. ๐Ÿ’ก What happens when LAIR1 is blocked Adding an antibody targeting LAIR1 (aLAIR1) disrupts macrophage-mediated inhibition. As a result CAR-T cells move more freely, they engage tumor cells more frequently, and tumor killing increases ๐Ÿง  Take-home message Tumors hijack macrophages to suppress immune attack and protect themselves. Finding immunotherapy targets that are ONLY expressed on cancer cells and NOT on healthy tissue is extremely challenging. So a company called A2 Biotherapeutics is designing whatโ€™s called Boolean logic-gated CAR-T cellular immunotherapies. A lot of cancers overexpress EGFR but because itโ€™s also expressed on healthy tissue it can be a challenging target. A2 Bioโ€™s Denali CAR-T trial is available to people with metastatic Triple Negative Breast Cancer. These CAR-T are designed to only attack EGFR-expressing cells if the cell is ALSO NOT expressing its HLA-A*02 molecule (HLA and MHC are the same thing but in humans we call it HLA). A2 Biotherapeutics has treated many people with this Boolean CAR-T strategy and at least one person with lung cancer had a complete response with their Mesothelin (MSLN) targeting CAR-T. CAR-T have rarely provided complete responses in solid tumor trials so this was a huge milestone. However an IG friend of mine was the first person treated with A2 Bioโ€™s EGFR-targeting CAR-T and she passed away approximately 6 weeks later. The trial, to the best of my knowledge, was not put on an FDA-hold following her death but itโ€™s important to know that these CAR-T trials are both cutting-edge and also carry significant risk. I always recommend people inquire about clinical trial related deaths. To qualify for the Denali trial, you first have to be screened through their Basecamp trial which assesses whether: 1) your healthy tissue normally expresses the HLA-A*02 type and 2) whether your cancer cells express EGFR and have lost the gene which codes for HLA-A*02 In order to qualify for the Denali trial, all three criteria have to be met in the Basecamp screening. If this type of proof of concept trial proves successful, the product could be extended to people with other HLA-A types. Some tumor genomics companies are now providing people with their basic HLA typing but the Basecamp trial would still be worth applying to to find out your cancerโ€™s EGFR expression levels and whether your tumor(s) have lost one of their HLA-A genes. Basecamp screening could be done well in advance of needing other options. #stageivneedsmore #tnbc #tnbcthriver
