Here’s why it matters: In cervical dystonia, the coordination between your eyes and your head is disrupted. Because of the unsteady head position, your eyes are constantly compensating, working overtime to stabilize your gaze, fix on targets, and make sense of where you are in space. When that system is overloaded, everyday tasks get harder. Reading. Walking in busy spaces. Driving. Following movement. Things that should be automatic suddenly take enormous effort. The good news? This coordination is trainable. Visual tracking exercises create an external focus that helps drive brain changes and you don’t need fancy equipment to get started (and they don’t have to be boring either!) → Punch balloon on a string or rubber band → Pickleball tracking → Standing racketball tracking → Balloon taps while walking → Wall ball toss and catch Ready to progress? 🚨 Skills Demo 🚨 Today we’re taking a look at cerebellar stroke rehab! 😵‍💫 Some common things to look out for with this type of stroke 🧠 1️⃣ Dizziness and/or nausea with changes in position (particularly going from supine to seated or standing) 2️⃣ Ataxia, a form of incoordination related to dysfunction with the calibration of movement patterns. Our patient will have difficulty knowing exactly how much force to exert when moving their body, resulting in a shaking pattern with poor balance, especially during ambulation! 🎶 It’s important to incorporate a regular use of a metronome when attempting gross movement patterns to help the brain recalibrate the force and rhythm of gross motor patterns 💪🏾 And it’s always great to focus on proximal strengthening and stability before addressing distal function, particularly fine motor skills. Using closed chain strengthening techniques with a metronome increases proprioceptive input which in turn helps calibrate movements. ✅ Build up the patients activity tolerance with upright positions over time and incorporate more challenging coordination patterns in opposing directions to continue to address the ataxia associated with cerebellar strokes! It takes time ⏰ but your initial rehab in the inpatient setting can be the difference between your patient walking with a device vs a wheelchair when they discharge! 🚶🏿

The patient in this video had a cerebellar stroke. Cerebellar strokes are rare and account for <3% of all stroke variances. The cerebellum, located at the bottom and backside of the brain, takes up the lion’s share of providing coordination to our bodies. Damage to a structure that is responsible for movement coordination will result in difficulties doing large movement tasks like walking, transferring, and climbing stairs, as well as fine motor tasks like dressing yourself, brushing your teeth, and driving. Ataxia is a specific medical term that describes this lack of coordination. The tasks I’ve chosen in this video are specific to restoring these deficits. We work on reaction based upper body coordination with an additional vector of pulling force via the cable attached to his waist that the patient must internally overcome to maintain his stability. We do many variations of this type of intervention. This video, like all the others, was created with the patient’s consent.
