Peripheral nerve blocks are a fascinating tool in headache medicine. Blocking the supraorbital nerve or the greater occipital nerve can reduce pain signalling from key trigeminovascular pathways involved in migraine and cluster headache. The mechanism is not simply local anaesthesia. These injections appear to modulate central pain processing and dampen sensitisation. Greater occipital nerve blocks in particular have reasonably good evidence. Studies show benefit in migraine frequency and intensity, and they are widely used as transitional therapy in cluster headache. If temporarily interrupting signalling at these points helps, could sustained mechanical stimulation or pressure (for example with a hair clip) have any meaningful effect? It’s biologically plausible and works for many of my patients though no one has officially studied it so there is currently no evidence to prove its efficacy. Remember only clip/stimulate the side that you get the headaches on. The clip above the eyebrow works best for supraorbital neuralgia/headaches more towards the forehead whilst the back of the head one works best for occipital neuralgia/cervical headaches/migraines that effect the back of the head more. Let me know if you’ve tried either of these and how you got on - if there are enough positive responses it might actually be worth doing a formal study. It’s not something you “push through,” and it’s not something you see from the outside. They’re diagnosed clinically, based on pattern and timing, and they affect the nervous system, sleep, and recovery more than people realise. In medical research, they’re not really understood - but they are diagnosed. When they show up, life stops as Marina could see! I cannot move, I’m debilitated in an instance. So I’ve made quite a few adjustments lately: 👉🏼 Training modified. 👉🏼 Recovery prioritised. 👉🏼 Sleep protected. Not as a setback, but management. I’m sharing this because strength isn’t pretending everything’s fine. Yesterday brought me to tears when I couldn’t complete what I set out to complete!
