STEP-BY-STEP PREPARATION for milled CAD/CAM ceramic crowns... The internally rounded shoulder is still the standard preparation margin in our courses…Repetition never hurts! I know this is BASIC, but I often see basic rules not followed, so here you go... People always ask: „HOW MUCH TOOTH REDUCTION?“ Well, that depends on the TYPE OF CERAMIC you want to use! Follow manufacturers instruction for minimum required thickness! It also depends on your (digital) WAX UP: what is the intended final shape/position of the tooth? And consider the abutment tooth color (based again on ceramic type)! DARKER TOOTH may require thicker ceramic and DEEPER PREP! ROUND AND SMOOTHEN the preparation for better FIT and STRENGTH! As mentioned in last post, the milling bur is NOT be able to mill very sharp corners (dependent on milling bur size and sharpness), which may lead to VOIDS and compromised fit (and possibly areas that are TOO THIN). It means reducing a tooth surface (usually the buccal or labial surface) in two different planes (angles) — to follow the natural contour of the tooth and maintain uniform crown thickness. ⸻ 🔹 In Maxillary Molar Crown Preparation ⚙️ Where it’s done: • On the buccal surface of the maxillary molar. ⚙️ Why: • The buccal surface of maxillary molars is not flat — it has: • Cervical third → more vertical • Occlusal third → more inclined toward the occlusal surface If you reduce it in one flat plane, you’ll either: • Over-reduce near the gingiva, or • Under-reduce near the occlusal surface. Hence, we use two planes to match the natural contour. ⸻ 🔹 Step-by-Step: 2-Plane Buccal Reduction 1. Mark Orientation Lines • Visualize or mark the tooth’s natural curvature — one near the gingival third, another near the occlusal third. 2. First Plane – Gingival Third • Hold the bur parallel to the long axis of the tooth. • Reduce the gingival third of the buccal surface (~0.8–1 mm). 3. Second Plane – Occlusal Third • Tilt the bur slightly toward the occlusal surface (following the tooth’s contour). • Reduce the middle and occlusal thirds (~1.2–1.5 mm for metal-ceramic crowns). 4. Blend Smoothly • Smoothly join the two planes to form a continuous, natural contour. • Avoid sharp junctions. ⸻ 🔹 Purpose of 2-Plane Reduction ✅ Maintains natural tooth contour ✅ Provides uniform thickness of restoration ✅ Avoids over-bulking of the crown ✅ Improves aesthetics and retention ⸻ 🔹 Tip 👉 For PFM crowns, 2-plane reduction ensures enough room for metal + porcelain layers without over-contouring the buccal surface.
