Hydrocephalus is enlargement of ventricles in brain.Normally CSF(water in the brain) is contained in Ventricles and regularly produced and absorbed.If there is any blockage of CSF flow pathway then proximal ventricles dilate and causes hydrocephalus.Hence causing enlargement of ventricles ,thinning or non development of brain and eventually head circumference increases. Opening pressure (normal 10-20 cm H₂O) rises in bacterial/TB due to edema, tumors or SAH. Protein (15-45 mg/dL) elevates non-specifically in inflammation; glucose (60% serum) plummets in bacterial/TB from microbial uptake. Cells decide: neutrophils (>80%) scream bacterial; lymphocytes point to viral/TB/autoimmune. Differentiation is pattern-based—bacterial meningitis: turbid CSF, very high pressure, massive neutrophilic pleocytosis (1000s/μL), protein >100 mg/dL, glucose <40% serum; rapid onset. Viral: clear CSF, normal/mild pressure, lymphocytic (100s/μL), mild protein rise, normal glucose; self-limiting. In geriatrics, low LP threshold prevents sepsis; correlate clinically for rapid therapy.
