The Importance of Removing Slack in Tourniquet Application A limb tourniquet is designed to stop life-threatening bleeding quickly. Research shows that removing any slack during the initial placement and clinching the tourniquet tightly before turning the windlass is crucial. If slack is present, it can require 2 to 6 times more turns of the windlass to achieve the necessary pressure to stop distal arterial flow. More turns can result in increased blood loss and poorer outcomes for the casualty. As demonstrated in the accompanying video, a tightly cinched Combat Application Tourniquet (C-A-T) achieved occlusion with just a three-quarter turn. To effectively stop life-threatening bleeding, continue turning the windlass of the applied tourniquet until the bleeding has ceased. Cinch the tourniquet tightly to minimize the number of turns and consequently reduce blood loss. How many times should you turn the windlass when applying a TQ?
