Extracorporeal Membrane Oxygenation (ECMO) is an advanced life-support technique used in critical care when the heart or lungs cannot function effectively. It temporarily oxygenates blood outside the body and returns it back to circulation, giving the patient time to recover.

DROne Wrong Step and It’s Game Over #lupus Extracorporeal Membrane Oxygenation (ECMO) is a life-saving medical intervention used when a person's heart and/or lungs are severely compromised and unable to provide sufficient oxygen to the body. ECMO works by temporarily taking over the function of these vital organs to support the patient's oxygenation and circulation. Here's how ECMO works: Cannulation: To initiate ECMO, one or more large tubes called cannulas are surgically inserted into the patient's major blood vessels. These cannulas serve as entry and exit points for blood. Blood Flow: The patient's blood is then pumped out of their body through one of the cannulas, typically from a vein, such as the jugular vein or femoral vein. The blood is drawn into the ECMO circuit. Oxygenation and Removal of CO2: Inside the ECMO machine, the blood passes through a membrane oxygenator. This device functions like an artificial lung, oxygenating the blood and removing excess carbon dioxide (CO2). Pumping: After oxygenation and CO2 removal, a pump in the ECMO machine sends the blood back into the patient's body through a second cannula, usually in an artery, such as the carotid artery or femoral artery. Support: The ECMO circuit continuously repeats this process, maintaining the patient's oxygenation and circulation. This provides time for the patient's heart and lungs to rest and heal. In some cases, ECMO can support a patient for days or even weeks. Monitoring: Throughout the ECMO treatment, the patient's vital signs, blood gases, and other parameters are closely monitored by a team of healthcare professionals, including perfusionists and critical care specialists. Weaning and Recovery: As the patient's own heart and lungs begin to recover and function adequately, ECMO support is gradually reduced, and the patient is weaned off the machine. Once stable, the cannulas are removed through a simple surgical procedure. ECMO is a complex and highly specialized intervention typically reserved for critical situations, such as severe respiratory failure or cardiac arrest. It can be a bridge to recovery or a bridge to a more advanced treatment, like a heart transplant or lung transpl

KILuca was four days old. I had been watching his stats all day, and it became very obvious his oxygen was decreasing and heart rate was increasing with each hour that passed, he was on maximum support. I knew what was coming.
Luca’s doctor sat us down and said “we are doing everything we can but the support we are offering Luca isn’t enough, he needs more, he needs ECMO”. He was calm, soft spoken, and gentle in the way he shared but there was also a sense of urgency. “If you want to move forward, we need to do it now, but this will be the only way we can save him”. It was 11:30pm. “Ok if it’s our only option, let’s move forward”. Moments later they were wheeling him down to a surgery room.
It was 1:00am the surgeon came in. His words and demeanor are something I’ll never forget. “Are you sure you want to do this?” He began reading off the list of risks like he was a server reading us items on a breakfast menu. “Brain bleed, stroke, brain damage, death…. If you make this choice he will be the sickest kid in the whole entire hospital, on his best day he is worse off than the sickest child here, do you understand that?” No I didn’t. I didn’t want to, how do I make this choice for my child that I met four days ago, it felt like too much responsibility.
I left the room and called my mom, I was in the corner of the hallway staring at room numbers, they were moving, I couldn’t breathe. If we don’t do it he dies, if we do it, there could be complications, he could suffer and he could die. This was a level of anguish I hadn’t ever experienced.
I went back to the room and talked to his doctor. “I don’t know what to do, how do we make a choice like this?” He looked at me with so much kindness, “if it was my child, I would do it, I think he will make it, I can’t promise you but I think the prognosis could be good”.
I took a deep breath, “ok let’s move forward”.
We were quickly escorted out and surgery began at 1:30am.
He made it through surgery and was responding to the machine that took his blood out of his body, added oxygen to it, and circulated it back into his body.
