After a Dementia Diagnosis… This Is What Most Families Aren’t Told After a dementia diagnosis, most families walk out with pamphlets… and a thousand questions afterwards. What many people aren’t told is that Medicare now has something called the GUIDE Model a dementia care management program designed to support not just the patient, but the caregiver too. When a provider participates, it can include: • A structured dementia care plan • Ongoing care coordination • Education about what’s coming • Help navigating community resources • Caregiver support sometimes even respite care. This isn’t just about confirming the diagnosis. It’s about managing the caregiving journey. This is not the entire solution. But it’s a starting point. If you’re navigating Alzheimer’s or dementia right now, comment below and tell me where you are in the process. And if you want practical, real-life guidance on aging, caregiving and planning ahead, visit us at AgeSmart411. @agesmart411 #alzheimers #agingparents #caregivingsupport #GUIDEModel #aginginplace

Yes it’s absolutely worth it and you better push your provider to be crystal clear about understanding the disease that is causing your dementia symptoms. Don’t accept “it’s dementia!” This is the time to add a neurologist to your care team because often this goes beyond your general primary care physician. For the month of March, I'm taking up space and sharing my story, my way. Once you've met one Alzheimer's caregiver, you've only met one Alzheimer's caregiver. Our journeys are wildly different—yet threaded together in ways that matter. In 2014, I was a young Black woman suddenly caring for my mother with early-onset Alzheimer's. I'm sharing our story so people just like me don't feel alone—but affirmed. Your story has power. And it connects you to more people than you realize. Follow along. Join us on the journey. #dementia #alzheimers #caregiver #millennial

Part 2 – because this gets overlooked. Sudden cognitive decline isn’t always dementia progression. UTIs, dehydration, poor sleep, and pain can cause severe shifts — and they often compound. What we saw wasn’t agitation. It was profound functional shutdown. Caregivers know when something isn’t right. Advocacy changes outcomes. Part 3 matters. Not every person with dementia needs memory care. Many individuals with Alzheimer’s disease or other types of dementia live in assisted living or nursing homes where their needs are fully supported. A dementia diagnosis alone does not automatically mean memory care is required. Here are five key factors to consider when choosing the right setting: • Wandering or exit-seeking. Memory care units offer secured environments that other settings may not. • Physical resistance during personal care. Memory care staff are often specially trained in de-escalation and dementia-specific behavior support. • Stage of dementia. Early stages may be appropriate for assisted living, while end-stage dementia often requires skilled nursing care. • Mobility. Someone who is fully immobile and not attempting to stand or wander may not need a secured unit. • Medical complexity. If skilled nursing services are needed, a nursing home or rehab facility may be more appropriate. Choosing between assisted living, memory care, and skilled nursing is highly individualized. It depends on safety, behavior, mobility, and medical needs, not just the diagnosis. #memorycare #assistedliving #nursinghome #dementiacare #caregiver

It can be confusing when your person with dementia suddenly says they need to go to work… even though they’ve been retired for many years. For them, this feeling is very real and usually connected to long-term memories, identity, and routine. In today’s video, I’m continuing my What To Do When… series and sharing what’s really happening in that moment and how you can respond in a way that reduces anxiety instead of creating an argument. These situations are incredibly common in dementia care, and the way we respond can make a huge difference. If you want more practical tips and strategies for navigating moments like this, put dementia 101 in the comments and I’ll send you the link to my Comprehensive Dementia 101 training. #dementia #alzheimer #memorycare #caregiver #joannadementiaexpert

How do you know it's dementia? My mom had some early signs and I knew something wasn't right. It wasn't just one or two, but several things that I noticed. She was young, and I started noticing some signs when she was in her late 50s but didn't realize then it was dementia. #dementia #caregiver

The dementia timeline that all people over 40 need to know 🧠 These signs can happen because dementia makes it harder for your brain to remember and pay attention. ➡️Losing focus on reading, work, or talking is your brain struggling to keep up. ➡️Feeling more anxious, irritable, or foggy happens because your brain gets tired handling normal tasks. ➡️Forgetting why you walked into a room shows short-term memory isn’t working as well. Noticing these changes early can help you get support and make life easier. Follow for more signs you might be seeing in your loved ones and learn how to help them. #dementiacare #caregivingsupport #seniorcare #caregiving #seniorassistance
