Let me tell you something about osteoporosis no one tells you. Sometimes the first sign is not a fracture. It’s a weird ache that doesn’t behave like a normal mechanical problem, burning through the mid back, a deep low back and hip pain that feels “off,” symptoms that move around and refuse to follow the usual rules. And yes, I’ve seen this show up before the osteoporosis cutoff, during osteopenia, and even earlier in life when the body is in a catabolic state. Bone is living tissue. When turnover shifts toward breakdown, the spine and hips can become less tolerant to normal life load. Microscopic stress can build inside vertebrae and the pelvis, and the nervous system can read that as pain. Vague. Deep. Confusing. This is where some people get misbucketed as “just chronic pain.” That doesn’t mean every chronic pain case is bone loss. It means bone health is often not screened when the pattern and risk factors fit. Quick DXA cheat sheet: T-score compares your density to a healthy young adult. Normal ≥ -1.0, osteopenia -1.0 to -2.5, osteoporosis ≤ -2.5 (for postmenopausal women and men over 50). A real de-identified case from our clinic: A 70-year-old woman had a Jan 2024 DXA showing lumbar spine osteoporosis (T-score -2.7). Two years later her Jan 2026 DXA improved to -1.7 with a statistically significant gain in bone density, and her total hip improved too (-2.4 to -1.9, statistically significant). No osteoporosis medications. The odd aches that brought her in settled as her physiology and capacity improved. If you’ve been told bone loss is inevitable, it isn’t. The path is simple but demanding, fix the biology (metabolic and hormone context, protein, nutrition, targeted supplementation when appropriate), then load the system progressively and safely with the right guidance.
