We’d argue that emergency medicine shines in approaching undifferentiated shock with a broad lens, then bringing in specialist teammates as needed. The next thing to do after delivering a shock is to RESUME COMPRESSIONS. Yep, that’s right. CPR. Don’t give in to temptation. Shock. CPR. 2 minutes. THEN pulse check. I’m sure I’ll catch some heat for this one… But let’s just get this out the way - I don’t think a slow code is ethically defensible And to be honest, I’ve never seen nor participated in the practice of “slow coding” So yes, I think there are real issues with legal / judicial decisions encroaching on the practice of good medicine including end of life care. And it’s unfortunate that the public has a skewed perception of what CPR can achieve at the end of a terminal illness. I also believe we need to take responsibility for training - really training - doctors how to have these sorts of conversations (and no, a “breaking bad news” module does not count as adequate training in high-stakes values based serious illness discussions). It’s unfair to both patients AND doctors to expect these conversations to go well when neither side has the necessary support In the meantime, my full rant was a minute too long to fit into a reel so keep your eyes on my YT page for the extended edition that includes questions to ask your medical team to better understand if a CPR would make sense for you #cpr #dnr #palliativecare #hospice #criticalcare #caregiving #healthcare
