DRImmune Thrombocytopenic Purpura (ITP)
Definition:
Immune Thrombocytopenic Purpura (ITP) is an acquired immune-mediated disorder characterized by isolated thrombocytopenia (platelet count <100 ×10⁹/L) due to autoantibody-mediated platelet destruction and impaired platelet production, in the absence of other causes of thrombocytopenia.
Epidemiology:
Commonest cause of isolated thrombocytopenia in children
Peak age: 2–7 years
Slight male predominance in young children
Often follows a viral infection or immunization
Pathophysiology (What actually goes wrong)
IgG autoantibodies target platelet surface glycoproteins (GPIIb/IIIa, GPIb/IX)
Antibody-coated platelets are destroyed mainly in the spleen
Autoantibodies may also suppress megakaryocyte platelet production in bone marrow.
Triggers:
Viral infections (URTI, EBV, CMV, varicella)
Post-vaccination (e.g. MMR)
Often idiopathic.
Clinical Features:
Bleeding severity correlates poorly with platelet count, but risk increases <20 ×10⁹/L.
Common manifestations:
Petechiae
Purpura
Easy bruising
Epistaxis
Gum bleeding
Menorrhagia (adolescents)
Serious but rare:
Gastrointestinal bleeding
Intracranial hemorrhage (<1%)
Key clinical clue:
👉 Child appears well, no fever, no weight loss, no hepatosplenomegaly, no lymphadenopathy.
Classification:
Acute ITP: <3 months (most children)
Persistent ITP: 3–12 months
Chronic ITP: >12 months (more common in adolescents).
Investigations:
ITP is a diagnosis of exclusion.
Essential:
CBC: isolated thrombocytopenia
Peripheral blood film:
Large platelets (young platelets)
Normal RBCs and WBCs
Not routine:
Bone marrow examination → only if atypical features
Coagulation profile → normal
ANA → only if SLE suspected.
Management (Child-centered, bleeding-based):
Observation only (most cases)
Platelets >20–30 ×10⁹/L
Mild or no bleeding
Child well
First-line treatment (if bleeding or very low platelets)
Corticosteroids
Prednisolone 1–2 mg/kg/day (short course)
IVIG
Rapid platelet rise (e.g. active bleeding, surgery)
Anti-D immunoglobulin (Rh-positive, non-splenectomized)
Emergency (life-threatening bleeding)
IVIG + high-dose steroids
