Visual Identification of Skin Lesions for Nursing Types of Skin Lesions (Clinical Dermatology) Understanding different skin lesions helps in early diagnosis of many dermatological conditions. 🔹 Macule – Flat lesion with only color change, no elevation (example: freckles). 🔹 Papule – Small solid raised lesion less than 1 cm (commonly seen in acne). 🔹 Nodule – Solid raised lesion larger than 1 cm, deeper than papule. 🔹 Vesicle – Small clear fluid-filled blister (example: chickenpox). 🔹 Pustule – Vesicle filled with pus, usually indicates infection. 🔹 Plaque – Broad, flat-topped raised lesion (commonly seen in psoriasis). 🔹 Scale – Keratin flakes present on the skin surface (example: dandruff). 🔹 Fissure – Deep linear crack extending into dermis (example: athlete’s foot). 🔹 Ulcer – Loss of epidermis and dermis, may heal with scar. 🔹 Scar – Fibrous tissue formed after healing of injury. 🔹 Erythema – Redness of skin due to vasodilation. 🔹 Cyanosis – Bluish discoloration of skin due to low oxygen (hypoxia). 🔹 Vitiligo – Well-defined depigmented patches caused by loss of melanocytes. 📌 Quick Tip: Primary lesions → Macule, Papule, Nodule, Vesicle, Pustule, Plaque Secondary lesions → Scale, Fissure, Ulcer, Scar ⚠️ Disclaimer: This content is for educational awareness only. Important Skin Lesions: → Macule Flat, non-palpable change in skin color without elevation or depression (e.g., freckles). → Papule Solid, raised lesion <1 cm in diameter, commonly seen in acne or inflammatory dermatoses. → Plaque Elevated, flat-topped lesion >1 cm, often formed by coalescence of papules (e.g., psoriasis). → Nodule Solid, deeper lesion involving dermis or subcutaneous tissue, usually >1 cm. → Vesicle Small, fluid-filled blister <1 cm (e.g., herpes simplex). → Bulla Large fluid-filled blister >1 cm (e.g., bullous pemphigoid). → Pustule Pus-filled lesion containing neutrophils (e.g., acne, bacterial infections). → Wheal Transient, edematous, raised lesion due to dermal edema (e.g., urticaria). → Comedo Plugged hair follicle seen in acne; may be open (blackhead) or closed (whitehead). → Papilloma Benign epithelial growth with a finger-like or verrucous surface. → Erythema Redness of skin due to vasodilation; blanches on pressure. → Telangiectasia Visible dilated superficial blood vessels. → Burrow Thin, linear track in the skin, classically seen in scabies. → Petechiae Pinpoint, non-blanching hemorrhages (<2 mm). → Purpura Larger non-blanching hemorrhagic lesions (2-10 mm). → Ecchymosis Bruise-like discoloration due to bleeding into the skin (>1 cm). → Hematoma Localized collection of blood within tissues due to vessel rupture. → Poikiloderma Combination of pigmentation changes, atrophy, and telangiectasia. #woundcare #awareness #SkinCare #healthcare #skinintegrity healthandwellbeing woundhealing fblifestyle WoundManagement socialmedia

Este es un caso de un carcinoma basocelular que se diagnosticó y trató quirúrgicamente de manera oportuna, logrando una cicatriz pequeña. Mándame mensaje si tienes alguna duda sobre una lesión. ACHANTOSIS NIGRICANS A 40 y.o, obese male with Fitzpatrick skin type IV and a history of persistent hyperinsulinemia presented with hyperpigmented patches on the axillae and neck. Dermoscopy revealed papillomatosis and hyperkeratosis, creating an uneven, velvety surface. These epidermal changes (ridges dan grooves) trap sebum, keratin, and cellular debris, resulting in dark spots or dots resembling open comedones, especially in moist intertriginous areas. The condition tends to worsen with excessive sweating and poor hygiene. #derm #dermoscopy #skinlesion #dermatology #dermoscopyexcellence #woodlamp #achantosisnigricans #dermatoscopy

“A common cause of hyperpigmented patches on the neck.” This patient presented with multiple asymptomatic hyperpigmented patches with fine branny (furfuraceous) scales over the neck. On gentle scratching, the fine powdery scaling becomes more apparent — a useful bedside clue. This presentation is typical of Pityriasis (Tinea) versicolor, a very common superficial fungal infection seen frequently in clinical practice. It is caused by Malassezia species, a lipophilic yeast that normally resides on human skin but can proliferate under favorable conditions such as: • Hot and humid climate • Excess sweating • Oily skin • Occlusive clothing The organism produces substances like azelaic acid, which interfere with melanin production, leading to hypo- or hyperpigmented patches with fine scaling. Diagnosis is usually clinical, but can be confirmed with: 🔬 KOH mount: showing short hyphae and spores (“spaghetti and meatballs” appearance). Treatment Most cases respond well to topical antifungals, such as: • Ketoconazole shampoo/lotion • Selenium sulfide lotion • Topical azole creams For extensive or recurrent cases, oral antifungals (itraconazole or fluconazole) may be prescribed. ⚠️ Even after treatment, pigment changes may take weeks to months to normalize. Prevention • Reduce excess sweating • Avoid tight occlusive clothing • In recurrent cases, periodic antifungal washes may help reduce relapse. #Dermatology #versicolor #ClinicalDermatology #SkinInfection #haldwani
