O PECARN é um protocolo médico usado no pronto-socorro para avaliar, de forma segura, se a criança tem risco de lesão grave na cabeça e se precisa de tomografia. Em palavras simples: ele ajuda a identificar quando observar é suficiente e quando investigar mais a fundo. ⚠️ Mesmo assim, fique atenta aos sinais de alarme após a queda: – Sonolência excessiva ou dificuldade para acordar – Vômitos repetidos – Choro inconsolável ou comportamento diferente do habitual – Convulsões – Perda de consciência, mesmo que rápida – Inchaço importante ou afundamento na cabeça A maioria das quedas é leve, mas vigilância nas primeiras 24 horas é fundamental. As principais indicações seguem protocolos como o PECARN (Pediatric Emergency Care Applied Research Network) e incluem: • Alteração do nível de consciência (Glasgow < 15). • Déficit neurológico focal. • Sinais de fratura de crânio • Vômitos persistentes (>2 episódios). • Convulsão pós-trauma. • Comportamento anormal ou irritabilidade excessiva (em lactentes). • Mecanismo de trauma de alto impacto Se a criança estiver bem, sem sinais de alerta, apenas observação pode ser suficiente

The PECARN network is at it again, stomping unnecessary pediatric cervical spine imaging this time. Here's what you need to know about the PECARN c-spine imaging decision tool for blunt trauma... They enrolled kids <18, 11857 in the derivation cohort, and 10573 in the validation cohort. Their rule had quite poor PPV (3-5%) and specificity (60%), but excellent NPV (99%) and sensitivity (93-94%). Consider CT if GCS 3-8, AVPU = U, abnormal ABCs (this one is not terribly helpful), focal neuro finding (weakness, paresthesia) Consider XR if any AMS, self-reported neck pain (also not real helpful), or major head/torso injury Overall, this is helpful to reduce the number of CTs quite a bit, but most will probably land in the moderate-risk zone at which time we will still need to make decision based on a fair bit of gestalt. I probably used this every day in EM and IM -Amboss: same idea as up to date, I would look up conditions, meds, diagnostics. (People mostly use it for the practice questions, but I would use the app bc it was great for referencing material) - Rosh: practice questions! -PA in a Flash & Cram the PANCE were such great reviews for EOR exams

Existe um protocolo chamado PECARN, validado em milhares de crianças, que ajuda a decidir quando há risco de lesão cerebral e necessidade de tomografia. 📌 O que aumenta risco: • alteração do nível de consciência • vômitos repetidos • sonolência excessiva • convulsões • dor de cabeça intensa • trauma significativo 📌 Se a criança está bem, ativa, sem sintomas → pode observar. 📌 Se aparecer qualquer sinal de alerta → precisa avaliação médica. Como neurocirurgião, eu te falo: não é sobre proibir o sono. É sobre saber quem está em risco.
