The reason it keeps coming back is because the piriformis is rarely the cause. It’s usually the reaction. When your hips and pelvis don’t move well together, your nervous system steps in and tightens muscles to protect you. The piriformis is one of the first to do that. So when you stretch it, you feel relief. But nothing changed in how your body is moving or loading that area. That’s why the pain returns. Sciatica sticks around when the system is the problem: • Poor hip control • Pelvis not sharing load • Spine doing work it shouldn’t be doing Until those pieces change, your body keeps using tension as protection. That’s why we start by teaching the hips and pelvis how to move independently and confidently again. Once the brain trusts the movement, the guarding shuts off. Less tension. Less nerve irritation.
