Travelling in a crowded train has been one of my biggest fears. After getting PPPD, I avoided trains completely for the past five years. But recently, I travelled for two days in a row with no issues. I felt the fear, but I controlled it and tried not to give any attention to the anxiety. After some time, everything felt normal. Today, I even stood in a crowded train for one straight hour. I’m proud of that. This may be a normal task for many people, but for someone with PPPD, it’s a big achievement and a real step toward overcoming fears. #pppd #vertigo #dizziness #earbalance #migraine

“What if I live like this forever?” “I’m never going to be able to work again” “I’m not going to be a good parent”......sound familiar? This is what’s happening: Chronic dizziness feels VERY out of control. Our bodies become totally unpredictable with symptoms and functionality. So, understandably, our brains try to gain some control over the situation....enter: rumination. Constant worrying gives our brains a faulty sense of control and this sense of control releases dopamine in the brain. Dopamine makes you feel motivated to continue doing that behavior, and the constant loop continues. The problem is that we cannot think our way out of uncomfortable feelings. The first step in calming this vicious thought cycle is to interrupt those thoughts. My favorite loop interrupter is: A - acknowledge that you’ve gone down the same loop C - connect with your body (maybe this is focusing on the breath or feeling proprioception) E - engage with your 5 senses to bring you back to the present moment. This requires A LOT of practice and in the beginning, you might feel like you spend all day trying to interrupt these thoughts. What’s your favorite method for stopping the thought loop? I’m Dr. Sina, chiropractor/global complex case consultant. PPPD (persistent postural-perceptual dizziness) is real. The problem is how often it becomes a catch-all label, and the workup stops. I see it constantly: chronic dizziness gets labeled “PPPD,” the plan becomes symptom coping for years (vestibular rehab, vision therapy, pacing, maybe meds), and if the patient plateaus everyone acts like the story is over. Plateau is not the end. Plateau is a diagnostic event. PPPD is criteria-based. If the classic pattern is not clear (symptoms most days for months, reliably worse with upright posture, motion, and visually busy environments, usually after a precipitating event), I do not accept it as the primary explanation. “Normal” vestibular tests also do not automatically equal PPPD. Here’s the blind spot: many persistent dizziness cases are not “just vestibular” and not “just psychological.” They are neck corridor cases. When I say “cervicogenic dizziness,” I’m not only talking discs and joints. I’m talking the neck corridor as a systems hub: • mechanics and proprioception (head-on-trunk input) • spinal cord and brainstem signaling (myelopathy patterns can present without neck pain) • arterial inflow and venous outflow patterns (positional intolerance matters) • nerve tension and thoracic outlet syndrome patterns (NTOS/VTOS style compression) • airway and breathing mechanics (sleep, CO2 tolerance, autonomics) • upper aerodigestive factors (throat, reflux patterns) that keep the system irritated Practical cues I refuse to ignore: 1. Dizziness reproducible with specific cervical positions. 2. Any abnormal neuro finding (gait change, reflex asymmetry, long-tract signs). 3. Symptoms that spike with head rotation/extension, or with sustained overhead arm use. 4. The “PPPD” patient who has done everything and still is stuck. I just wrote up a case where a PPPD diagnosis persisted for years, until a full exam and systems screen exposed the real driver.
