The set was built to create the illusion of a reflection, including a staircase that was physically flipped so the trick begins well before the bathroom scene. The slow motion in the hallway was not added in post. The camera speed was ramped live during the shot to create the feeling that Ellie cannot reach the medicine in time to save her father. The shot was planned months in advance and executed using a VistaVision camera on Steadicam with an extremely coordinated crew move.
