It can happen after an injury, emotional trauma, or due to long-term pain, causing your body to “overreact” to even normal sensations. Factors That Drive Central Sensitization in Chronic Pain 1. Triggering factors Injury, surgery, repeated flare-ups, infection, chronic stress, or trauma. Each one tells the nervous system: Protect at all costs. The pain alarm becomes sensitive, and the brain associates pain with threat. 2. Maintenance factors Fear of pain, hypervigilance, poor sleep, inflammation, emotional suppression, and social invalidation. These reinforce the sense of danger. Over time, the spinal cord and brain amplify standard signals, turning up the “volume knob” on pain. 3. The loop Neurons fire more easily. Descending inhibition weakens. Pain circuits become overlearned. This is central sensitization when protection becomes pain. 🧠 And yet, part of healing is remembering that protection was care. Your brain wasn’t broken; it was trying to keep you safe. 𝗖𝗲𝗻𝘁𝗿𝗮𝗹 𝘀𝗲𝗻𝘀𝗶𝘁𝗶𝘇𝗮𝘁𝗶𝗼𝗻 signifies slowly developing plastic changes in the brain because of long-standing inflammation or disease state in the body or a feeling of a 𝘭𝘢𝘤𝘬 𝘰𝘧 𝘴𝘢𝘧𝘦𝘵𝘺 in the the psycho- social context, making the brain to act like an overprotective helicopter parent, who wants to know everything happening in the body as well as in the environment.⁣ ⁣ 👉 Let’s say this is the S2 level of spinal cord where nerves from the pelvic region bring info to be relayed to the brain.⁣ ⁣ ✅ The 𝗔-𝗯𝗲𝘁𝗮 nerves bring info of light touch, example wearing an underwear ,while ⁣ ✅ The 𝗖 𝗻𝗲𝗿𝘃𝗲𝘀 bring info of unpleasant sensations called nociception I.e unpleasant pressure, touch, temperature & chemical changes (inflammation). ⁣ ✅ S2 level also gets information from the other levels above and below it that is S1, L5, S3 etc. ⁣ ⁣ 👉 All this info gets blocked by the 𝗶𝗻𝘁𝗲𝗿𝗻𝗲𝘂𝗿𝗼𝗻𝘀 in the spinal cord that act as a spam filter so the brain doesn’t get bothered unnecessarily and only receive relevant and important information that helps the brain keep everything in order and take action in a very specific, measured way.⁣ ⁣ 👉 To further make sure, brain also releases 𝗱𝗮𝗺𝗽𝗲𝗻𝗶𝗻𝗴 𝗰𝗵𝗲𝗺𝗶𝗰𝗮𝗹𝘀 such as dopamine, serotonin, endorphins to suppress this extra information to maintain a state of calm. ⁣ ⁣ 🥵 But in central sensitization the interneurons in the spinal cord die due to inflammatory changes brought on by 𝗖 𝗻𝗲𝗿𝘃𝗲 fibers. Noticing the danger, brain also stops making the dampening chemicals as it seeks more info to plan its next step to thwart any danger at any time. It’s war time!! 🥷⁣ ⁣ This results in:⁣ 1. Continuous barrage of info from A beta fibers e.g. the underwear is touching the vulv@/ scrotum.⁣ 2. The unpleasant info won’t stop ever & brain keep feeling that the injury to the tissue is persisting.⁣ 3. Central sensitization is a feature of chronic pain where the nervous system becomes more alert to various sensations. For low back pain, these sensations can be many different things like movements, loading, stress, touch, temperature, etc. The common approach for chronic low back pain is to use core exercises, which are low intensity exercises for the muscles around the trunk. Common exercises include: planks, side planks, glute bridges, dead bugs, bird dogs, etc. But a recent research article found that high intensity exercises were beneficial in reducing central sensitization. Which means that potentially loading with heavier exercises would create a larger stimulus needed to help those experiencing chronic pain. Obviously this doesn’t mean loading as heavy as possible ignoring the load tolerance of the person. But re-consider the exercises commonly used for the treatment of chronic low back pain to emphasize heavier loads within tolerance.

Today we will break down central sensitization. 🧠CENTRAL SENSITIVITY MECHANISMS: Neuroplasticity: Changes in synaptic strength and reorganization of neural networks. Wind-Up & Altered Pain Processing: Enhanced response to pain signals and reduced inhibition of these signals. Glial Activation: Glial cells in the CNS become activated and release pro-inflammatory cytokines, contributing to pain signaling. ——————— 🧠CENTRAL SENSITIVITY SYMPTOMS Hyperalgesia: Increased sensitivity to painful stimuli. Allodynia: Pain in response to normally non-painful stimuli.
