THSGPT vs SGOT – interpretation
• SGPT (ALT) = Serum Glutamate Pyruvate Transaminase → also called Alanine Aminotransferase.
• SGOT (AST) = Serum Glutamate Oxaloacetate Transaminase → also called Aspartate Aminotransferase.
• ALT is more liver-specific, mainly present in hepatocytes → better marker of liver injury.
• AST is less specific because it is present in liver, heart, skeletal muscle, kidney, brain, and RBCs.
• Normal Range
ALT: 7–56 U/L
AST: 8–40 U/L
• ALT elevation commonly seen in viral hepatitis, fatty liver, drug-induced liver injury.
• AST elevation can occur in liver disease, myocardial infarction, muscle injury, and hemolysis.
⭐ Exam Pearl (De-Ritis Ratio)
• AST / ALT > 2 → Alcoholic liver disease
• ALT > AST → Viral hepatitis / acute liver injury
⚠ Exam Trap
• AST may increase in myocardial infarction and muscle damage, so high AST does not always indicate liver disease.
