It is mainly used for severe depression with suicidal tendency and treatment-resistant depression. 3. Other indications include catatonia, mania, schizophrenia with catatonic features. 4. Absolute contraindication: Raised intracranial pressure (ICP). 5. Relative contraindications: Recent MI, uncontrolled hypertension, aneurysm, severe cardiac disease. 6. Modified ECT is standard—given with anesthesia and muscle relaxant (succinylcholine). 7. Procedure: NPO 6–8 hrs, consent, premedication, electrode placement, current 70–120V. 8. Bilateral ECT works faster, but causes more memory loss; unilateral has fewer cognitive effects. 9. Side effects: Headache, confusion, transient memory loss, retrograde amnesia. 10. ECT still remains one of the most used treatments for severe, treatment resistant depresson, but there are newer options available today. 1) Repetitive Transcranial Magnetic Stimulation (rTMS): This uses magnetic pulses to activate mood related areas of the brain. During the procedure, controlled electric currents are passed through the brain while the patient is under general anaesthesia, intentionally triggering a brief seizure. This process is believed to cause changes in brain chemistry that can rapidly relieve symptoms of mental illness when other treatments, such as medication or therapy, have not been effective. Though ECT has a controversial history, modern techniques are much safer and more refined, with close medical supervision and significantly reduced side effects. According to **Swaiman's Pediatric Neurology**, only about 10% of children have tumor or stroke, and in one large study only 4 out of 613 epileptic children had a brain tumor. Many pediatric focal seizures are self-limited polygenic disorders like **Benign Rolandic Epilepsy**. ⚠️ Important Clinical Pearl: In children, “loss of awareness” is hard to assess — especially in preschool or preverbal age. Lack of response ≠ impaired consciousness. Yes, we are medicated and I’ve already spoke with his doctor. I couldn’t imagine being in his body. Feeling like you don’t have control. That’s a lot to take in especially for a 6 year old.
