Ocular ischemic syndrome (OIS) comprises the ocular signs and symptoms attribut­able to chronic ocular hypoperfusion secondary to severe occlusion of the internal carotid artery. Ocular findings may be the only manifestation of carotid occlusive disease, although half of patients with OIS also have severe cardiovascular disease. Five-year mortality among patients with OIS is around 40%, with two-thirds of the deaths caused by myocardial infarction and one-third by stroke. Thus, timely diagnosis is necessary to reduce cardiovascular morbidity and mortality as well as to prevent permanent vision loss. Differential diagnosis. OIS is most frequently misdiagnosed as central ret­inal vein occlusion (CRVO) or diabetic retinopathy. Vision loss is typically acute in CRVO and chronic in diabetic reti­nopathy and OIS. In CRVO, the veins are both dilated and tortuous, unlike OIS, in which the veins are dilated but not tortuous. Dilation of veins is often attributed to an inflow obstruction, while the tortuosity seen in CRVO is chiefly due to an outflow obstruction. Retinal hemorrhages are often present throughout the retina in CRVO and are most prominent in the posterior pole in diabetic retinopathy. In contrast, the retinal hemorrhages in OIS are most prominent in the midperiphery. Because OIS causes ischemia of the long and short posterior ciliary arteries and of the anterior ciliary arteries , it often leads to anterior segment ischemia. Therefore, signs of choroidal or anterior segment ischemia, such as anterior chamber cells and flare (and, rarely, keratic precipitates) or choroidal nonperfusion seen on FA or ICGA, may be present in OIS but not in CRVO or diabetic retinopathy. . . #eye #medicallife #ophthalmology #optometry #eyecare

It is caused by complement activation after acute crushing injuries to the head or thorax. ▪️Purtscher's retinopathy typically follows trauma in the absence of direct trauma to the globe. ▪️Clinical presentation is variable, with some patients presenting with profound vision loss and others with minimal changes. ▪️A relative afferent pupillary defect may or may not be present. ▪️Clinical findings are cotton wool spots, retinal hemorrhages, and Purtscher's flecken, which are areas of whitening in the inner retina between the retinal arterioles and venules. ▪️OCT displays hyperreflectivity of the inner retinal layers due to inner retinal edema. ▪️Prognosis is variable and largely depends on the extent of retinal involvement. ▪️Similar-appearing Purtscherlike retinopathy can be caused by a variety of etiologies including acute pancreatitis, chronic renal failure, autoimmune diseases, fat embolism, amniotic fluid embolism, retrobulbar anesthesia, and orbital steroid injection. When the strip is moistened with normal saline and touched to the conjunctiva, it releases a bright green dye that fluoresces under cobalt blue light. Uses of Fluorescein Strip in Eye 1. These vessels are part of the central nervous system and are uniquely visible because the retina is transparent, allowing light to pass through and illuminate the blood vessels beneath. This direct view makes the eye an important diagnostic window into overall health. Changes in the size, shape, or appearance of retinal blood vessels can reveal early signs of conditions such as high blood pressure, diabetes, atherosclerosis, and even stroke risk. Swelling of the optic nerve, tiny hemorrhages, or narrowed arteries can appear in the eye before symptoms show up elsewhere in the body, which is why routine eye exams can sometimes uncover serious systemic diseases. While advanced tools like capillaroscopy can visualize blood flow in tiny surface vessels near the fingernails, these require specialized equipment and are not part of standard physical exams. In contrast, retinal blood flow can be assessed quickly and non invasively during a routine eye checkup. This makes the eye uniquely valuable in medicine, serving as the only place where the circulatory system can be directly observed in action using everyday clinical tools.
