PPPD vs. cervicogenic dizziness 💫 Not all dizziness comes from the same system… This is why treating the wrong one can keep you stuck. PPPD (Persistent Postural-Perceptual Dizziness) This shows up as daily dizziness and feeling worse when you move, turn your head, or walk through busy environments. In PPPD, the brain relies too much on vision for balance instead of body and inner ear input. That mix-up makes the world feel weird, even if it’s not spinning. That’s why these exercises use eye tracking and head movement. They help teach the brain how to use the eyes and body together again so balance feels steady. Cervicogenic Dizziness This kind of dizziness often comes with a stiff or sore neck. Your neck has sensors that tell your brain where your head is. If those sensors aren’t working well, your the neck sends unclear or distorted messages to the brain… So the brain gets confused and you feel dizzy. That’s why these exercises focus on the neck and posture. When the neck sends clearer messages, balance gets better. #doctoradvice #pt #dizziness #dizzy

“What if I live like this forever?” “I’m never going to be able to work again” “I’m not going to be a good parent”......sound familiar? This is what’s happening: Chronic dizziness feels VERY out of control. Our bodies become totally unpredictable with symptoms and functionality. So, understandably, our brains try to gain some control over the situation....enter: rumination. Constant worrying gives our brains a faulty sense of control and this sense of control releases dopamine in the brain. Dopamine makes you feel motivated to continue doing that behavior, and the constant loop continues. The problem is that we cannot think our way out of uncomfortable feelings. The first step in calming this vicious thought cycle is to interrupt those thoughts. My favorite loop interrupter is: A - acknowledge that you’ve gone down the same loop C - connect with your body (maybe this is focusing on the breath or feeling proprioception) E - engage with your 5 senses to bring you back to the present moment. This requires A LOT of practice and in the beginning, you might feel like you spend all day trying to interrupt these thoughts. What’s your favorite method for stopping the thought loop? Travelling with PPPD wasn’t easy. After I developed PPPD, I started fearing going out. Every time I stepped outside, anxiety would hit me. I kept imagining all the worst possibilities — especially the fear of getting dizzy again. Because it happened once… and then it happened again… My brain started treating going outside as a danger. This turned into panic attacks, fast heartbeat, and avoidance. So I stayed in my comfort zone. But later I realized — the comfort zone was the real prison. Your brain keeps sending the same old warning signal because of past experiences… But that doesn’t mean the danger is real. So I started again. Small walks. Short travels. Yes, the dizziness came. Yes, the anxiety came. But I kept going. Again and again. And something changed. The trigger didn’t control me anymore. The intensity slowly reduced. And today, I travel again. Sometimes the dizziness comes — but I don’t give it attention anymore. Because life outside is bigger than the fear inside. I’ve seen the worst PPPD can do — and I’m still here. Still moving. Persistent Postural-Perceptual Dizziness (PPPD) is a chronic dizziness disorder where the brain becomes overly sensitive to motion, posture, and visual stimuli. It’s one of the most common causes of long-lasting dizziness, yet many patients go months before getting the diagnosis. What PPPD feels like Unlike vertigo, PPPD usually does NOT cause spinning. Patients often describe: • Feeling rocky, swaying, or like they’re on a boat • Constant or daily dizziness lasting >3 months • Worse when standing or walking • Worse in visually busy environments (grocery stores, crowds, scrolling screens) • Symptoms triggered by head movement or motion around them What triggers PPPD PPPD usually starts after another event affecting balance, such as: • Benign Paroxysmal Positional Vertigo (BPPV) • Vestibular neuritis • Labyrinthitis • Concussion • Severe panic or anxiety episode The original problem improves, but the brain remains stuck in a hyper-alert balance state. Key diagnostic clues Providers start thinking about PPPD when: • Dizziness lasts >3 months • Symptoms are present most days • Worse when upright, moving, or visually stimulated • Neurologic and ear exams are usually normal Treatment The goal is retraining the brain’s balance system. Evidence-based treatments include: • Vestibular rehabilitation therapy • Gradual exposure to motion/visual triggers • SSRIs or SNRIs in some patients • Managing underlying anxiety or vestibular hypersensitivity The good news PPPD is very real and very treatable. Once recognized, most patients improve significantly with the right therapy. #pppd #vestibulardisorder #healthconditions #nurse #nursepractitioner
