Don’t forget to follow for more 👉🏼 @rev.med ✨ Intrinsic Muscles of the Shoulder ✨ ✅ Overview Shoulder muscles move the upper limb and produce the shape we see of the shoulder. We can divide them into extrinsic muscles (means they originate from the torso, and attach to the bones of the shoulder) and Intrinsic (means they originate from scapula/clavicle and attach to the humerus. Intrinsic group has 6 muscles:  *Deltoid  *Teres major *Four rotator cuff muscles (swipe the post above to read the function, innervation, origin, & attachment) *Deltoid Its like an inverted triangle and has 3 parts (anterior, middle & posterior) Origin: Lateral third of the clavicle, acromion & spine of the scapula Insertion: Deltoid tuberosity of the humerus  Function:  -Anterior part: flexion & medial rotation -Middle part: major abductor of the arm (after supraspinatus does the first 15 degrees.. deltoid takes over) Posterior part: extension & lateral rotation  Innervation: Axillary nerve *Teres Major If you know about the ‘Quadrangular space’ well this muscle forms the inferior border of it. The Hidden Culprit Behind Your Arm Pain: The Back of Your Shoulder Blade Most people think shoulder pain starts at the joint—but it often begins behind the shoulder blade, where critical nerves and blood vessels pass through tight anatomical corridors. When these areas get congested, symptoms like arm weakness, deep aches, tingling, or even muscle wasting can follow. Behind the shoulder blade are three spaces where neurovascular structures are vulnerable: • Quadrangular Space: Contains the axillary nerve and posterior circumflex humeral artery. Compression here (especially in desk workers or dentists) can cause deltoid weakness, teres minor atrophy, and numbness in the lateral shoulder. This is called Quadrangular Space Syndrome. • Triangular Interval: A passage for the radial nerve and deep brachial artery. In bodybuilders, hypertrophy of the teres major or triceps can narrow this space, leading to radial nerve compression, wrist/finger weakness, and triceps fatigue. • Triangular Space: Houses the circumflex scapular artery. While it doesn’t carry a major nerve, congestion here contributes to global scapular tension and impaired circulation. We see this all the time at Movability—in truck drivers slouched for hours, dentists hovering over patients, office workers reaching for their mouse, and athletes with massive upper bodies. The fascia in this region acts like cling wrap. When it tightens, it can strangle nerves and vessels, causing chronic pain and functional decline. If caught early, we can prevent this from progressing. But once nerve compression sets in, recovery takes time and expert care. That’s why we use myofascial release and neuromuscular decompression therapy to open up these spaces, relieve pressure, and restore function. Combined with posture correction and ergonomics, it’s one of the most effective ways to treat and prevent these syndromes before they cause irreversible damage. If your job depends on your arms, this kind of maintenance isn’t a luxury—it’s a necessity. – Dr. Sina

Day 5/50 In the axilla, the axillary nerve is located posterior to the axillary artery and anterior to the subscapularis muscle. It exits the axilla at the inferior border of subscapularis via the quadrangular space, often accompanied by the posterior circumflex humeral artery and vein. The axillary nerve then passes medially to the surgical neck of the humerus, where it divides into three terminal branches: Posterior terminal branch – provides motor innervation to the posterior aspect of the deltoid muscle and teres minor. It also innervates the skin over the inferior part of the deltoid as the upper lateral cutaneous nerve of the arm. The anterior terminal branch – winds around the surgical neck of the humerus and provides motor innervation to the anterior aspect of the deltoid muscle. It terminates with cutaneous branches to the anterior and anterolateral shoulder. Articular branch – supplies the glenohumeral joint Spinal roots: C5 and C6. Sensory functions: Gives rise to the upper lateral cutaneous nerve of the arm, which innervates the skin over the lower deltoid (‘regimental badge area’).
