Googling “scleroderma” is terrifying — because it sounds like one diagnosis. It’s NOT. Scleroderma is an umbrella term, not a single disease. There are localized forms like morphea or linear scleroderma, which affect the skin and underlying tissue and do not involve internal organs. There are also systemic forms, called systemic sclerosis, which include limited disease (often called CREST) and diffuse disease. These are the forms that can involve internal organs and require closer monitoring. This distinction matters — because prognosis, testing, and treatment look very different depending on which type someone has. As a rheumatologist, I don’t diagnose scleroderma based on one lab. When systemic sclerosis is suspected, I look at: • Symptoms and physical exam (skin thickening, Raynaud’s, joint findings) • Antibody testing such as anti-centromere, Scl-70, RNA polymerase III • Inflammatory markers and baseline labs (CBC, CMP, ESR/CRP) • Lung screening when appropriate (PFTs ± HRCT) • Patterns and changes over time Google often shows worst-case scenarios. Before & after my Scleroderma diagnosis 🌻 I’m going to make this a short post because it has been one of the hardest videos to make. On the positive side it is good reminder that regardless of my Scleroderma diagnosis my body has been constantly changing. We tend to get attached to our looks forgetting that we are not going to be young forever. We are all going to change as we get older 😜 My transformation is just a little bit different. Systemic Scleroderma is an autoimmune disease that attacks the connecting tissue. Most of your body is made up of connective tissue so that means it can attack 99% of your body. Not everyone has the same progression. Some have more internal issues and others more external. The thing is that change is inevitable so it’s better to accept the reality of your condition than to keep fighting it. I think the saddest part about this post is knowing that the woman before my diagnosis was so insecure about her looks. I wish I could go back and hug her and let her know she was and is still beautiful 💕🌺 & she was stronger than she thought she was. Now I’m here and I’m still trying to get used to my new looks but I’m not angry anymore. Mixed connective tissue disease is an overlap disease that can have features of inflammatory myositis, lupus, systemic sclerosis, interstitial lung disease, AND high titer ANA and RNP.

If you notice any of these symptoms, it's important to see your rheumatologist as they could be early signs of systemic sclerosis: • Swollen, puffy fingers and/or swelling in the hands that doesn't leave a dent when pressed. • Thickened skin, which can affect areas like the hands, feet, face, and forearms. • Tightened skin around the mouth, making it harder to open fully. • Indentations (pitting) on your fingertips, along with tissue loss and small sores near your nails. • Fingers that are swollen and stiff, joint and muscle pain, and feeling unusually tired. Systemic sclerosis is a complex disease with a wide range of symptoms and outcomes. It affects different organs and can progress differently in each person. Skin problems are very common in systemic sclerosis. People with this condition often experience thickening and hardening of their skin to varying degrees. The fingers, hands, and face are usually the first areas affected. Almost all patients with systemic sclerosis have Raynaud's phenomenon. Raynaud's phenomenon causes temporary spasms in the blood vessels of the hands and feet due to changes in how the arteries work. Over time, many patients with systemic sclerosis develop permanent changes in their small blood vessels, which can lead to longer episodes of Raynaud's phenomenon. This can cause pain, sores on the fingers and toes, and in severe cases, serious circulation problems. Up to half of patients develop sores on their fingers and toes due to poor circulation.
