Intraocular pressure check: This device is called a rebound tonometer. Instead of tapping the device on the cornea like other tonometers, this device uses a disposable plastic ball mounted on a stainless steel stem. The stem is held in place by an electromagnetic field. When the button is pressed, a spring propels the stem toward the cornea. The speed at which it decelerates is determined by intraocular pressure. The speed is measured and converted to a pressure that is displayed on the screen. Unlike other tonometers, the entire device does not touch the cornea. This makes it more comfortable for patients and eliminates the need for anesthetic drops. It also does not use a puff of air, which is difficult for children. This particular model needs to be held parallel to the floor and at the correct distance. There is a mechanism that rests on the patient’s forehead to help maintain distance (not shown in this video), and the light is green when the device is level. The measurement taken in this slow-motion video was 11 mmHg, which is normal. The machine is measuring the patients eye pressure! A Noncontact tonometer blows a puff of air on the eye and measures the time from initial generation of the puff of air when cornea gets flattened (in milliseconds) to the point where the timing device stops and uses this to calculate the eye pressure. The air puff flattens the patient's cornea in a non-invasive way, meaning eyes do not need to be numbed prior to the test. Pregunta frecuente: * Hay dispositivos que pudiera tener el paciente en la casa para medirla ?. R/: Si los hay pero presentan varios desafíos: 1. Su costo 2. Si el paciente puede asistir a sus controles oftalmológicos periódicamente , su enfermedad está estable y su presión ha permanecido controlada realmente no se justifican. 3. La presión intraocular no es el único factor que miramos durante el seguimiento y el hecho de encontrarla aparentemente bien los podría confundir y descuidarse en su seguimiento. 4. Muchos de los pacientes no miden correctamente su presión 5. El dispositivo gold standard o patrón de oro para medir la presión intraocular es el tonometro de Goldman, que requiere aplicación de anestésico y un colorante amarillo . Este requiere estar adaptado a una lámpara de hendidura. Esto se hace solamente en el consultorio !. * Entonces Que pacientes se beneficiarían de tomar la presión ocular en casa ?. R/: Aquellos que por algún factor no pueden asistir a la consulta por ejemplo, quienes tienen limitaciones físicas . * Pero y si mi presión fluctúa y cuando voy a consulta el doctor me la encuentra bien ?. R/: Si el doctor sospecha que estás teniendo picos de presión y considera necesario te realizará una curva de presión en el consultorio y podría realizarte una prueba con una carga de agua .
