EPAFE (amniotic fluid embolism) is a rare, life-threatening obstetric emergency that presents with sudden cardiovascular collapse followed by DIC (disseminated intravascular coagulation). Severe pulmonary vasoconstriction from inflammatory mediators prevents the right ventricle from pumping blood into the lungs and out to the body via the left ventricle, resulting in cardiac arrest.
AFE is more common in patients of advanced maternal age, and those undergoing operative delivery. There is a strong association with abnormal placentation (placenta previa / placenta accreta).

Nearly 16 years ago, on March 31, 2010, during my son’s birth, we both stopped breathing.
It took life-saving measures to bring us back.
I remained in a coma for 3 days and 4 nights and on Easter Sunday, April 4, 2010, I was given a second chance at life. 🙏🏽 A resurrection miracle ✝️
I truly believe God was with us that day. It was His will that we return to the living.
Many mothers and their babies don’t get that chance—about 80% do not survive.
By God’s grace, I am part of the 20% who survive an amniotic fluid embolism… and Mateo is our miracle. 💙

In honor of AFE Awareness Day, I’m sharing my “never” experience with AFE, and how that moment has forever shaped me as a nurse.
AFE can make us feel so powerless, but there are things that we can do to ensure that we are prepared if and when we fall on the side of experiencing that “never” event.
One of the many incredible things that the AFE foundation provides is FREE education to healthcare providers to ensure that we have the skills and tools to be able to know and recognize the signs early so that we can act with intention.

BI🩵 Today is AFE Awareness Day 🩵
7 years ago (to the day) when we announced that our daughter was going to be a big sister we had no idea what an #AFE was.
While in labor I suddenly awoke from a nap and shouted to my doula @mission_britt that my heart was racing.
Then, Eli went to get my nurse @runner_kristi. By that time they returned I went into cardiopulmonary arrest. Praise the Lord my daughter was rushed to one of the best NICU’s in the state and she has no long term deficits.
During my treatment I would need @laur_espresso’s awesome #ECMO team, 7 surgeries, 14 days on a ventilator, 6+ years of rehab to relearn the most basic skills like eating, writing, reading.
Even with how hard this road has been I would choose it a thousand times over as I got to come home to my family.
So many moms and babies do not make it home after this catastrophic complication.
This is why funds for research are paramount.
This is why educating your on up to date treatments are key. **Spoiler Alert…it’s not AOK**

Amniotic Fluid Embolism is one of the most fatal birth complications in the world.
Published rates of survivability range from 20-60%.
1 in 40,000 families will experience this catastrophic complication in the U.S.
That’s approximately 100 families per year.
Amniotic fluid embolism (AFE) is a sudden and unexpected life-threatening birth complication that can affect both mother and baby. Although poorly understood,It is believed to occur when a mother suffers an allergic-like (immune) response to amniotic fluid and fetal material that enters her bloodstream during labor or shortly after delivery. It is important to note that the entrance of amniotic fluid and fetal material into the bloodstream is a normal part of the birth process and in most women, it does not cause this serious reaction.
Most often an AFE involves two very serious and life-threatening complications; heart and lung failure (cardiorespiratory collapse) and severe bleeding (disseminated intravascular coagulopathy or DIC). Independently, each of these complications is extremely serious and life-threatening. Together, they are exceptionally challenging to treat and require immediate and aggressive medical care.
We know so much more than we did 15 years ago.
But, we still don’t know why this happens. We still don’t know who it will happen to.
We still have so much more work to do.
