Protein shakes are probably the #1 product sold to bariatric patients. Every clinic recommends them. Every supplement company pushes them. Most of us leave surgery with an entire cabinet full. But here’s the part that gets confusing. Protein shakes are incredibly helpful early post-op. You can’t eat enough volume yet. Your stomach is healing. And you still need to hit protein goals to protect muscle and support recovery. Guidelines from the American Society for Metabolic and Bariatric Surgery recommend about 60–80g of protein per day, which is hard to get through food in those early weeks. So shakes make sense in the beginning. But they were never meant to be your forever diet. Most shakes are ultra-processed and often contain artificial sweeteners, fillers, and highly processed protein isolates. And research has shown liquid calories don’t create the same fullness as solid food, meaning they can leave you feeling less satisfied than real meals. Studies in The American Journal of Clinical Nutrition have shown people often consume more calories later after liquid meals compared to solid ones. WHY do you need so much protein after bariatric surgery? Because your stomach size is severely restricted, it’s very difficult to physically eat enough food to get the protein you need. Also, some procedures (like the gastric bypass or duodenal switch) can reduce your body’s ability to absorb nutrients, including protein, from the small amount of food you do eat. The high goal of 60-100 grams is set to compensate for both the limited food volume and potential malabsorption, making sure that even with these restrictions, your body still gets a minimum of what it requires. This is why protein supplements (shakes, powders) are so heavily relied upon, especially in the first few months, as they are a high-dose, easy-to-digest source of pure protein. It is achievable by being intentional and focusing on repeating meals that you know are effective. Most people that hit their goals are eating the same thing over and over. At 7 months post-op, I hit a point nobody warned me about. I was doing everything “right.” Protein shakes. Protein bars. Sugar-free drinks. All the “high protein bariatric foods.” But my food noise was a volume 11. Constant cravings. Always thinking about food. Hungry in a way that didn’t make sense. So I started looking closer at what I was actually eating. Almost all of it was ultra-processed. Protein powders. Artificial sweeteners. “Protein snacks.” So I ran a little experiment.
