A challenging trimalleolar ankle fracture dislocation. We did require three plate fixation and combination of cannulated screws. When fixing these via posterior approach, it is always important to release periosteum around fractures to be able to mobilize them back to anatomic alignment. Once posterior malleolus is free, go and get the fibula out to length, pin it and place a lag screw if possible and go back and place an anti-glide plate for the posterior malleolus fracture. This is so you can visually see if the fracture is out to length or to ensure plate isn’t too low on lateral c-arm image. This can be difficult to see if you plate the lateral malleolus first. Next, plate the fibula. Lastly fixate the medial malleolus with screws but in this case a hook plate was also necessary. Doing this while still prone is challenging but can be done with patience. Lastly stress the ankle to see if syndesmosis or any ankle ligaments needs to be fixated. Hope these tips help with your next case. #anklefracture #podiatry #orthopedics #surgeon #medicine

Partial knee replacement is not a “smaller total knee.” It’s actually a precision operation in a very small space. I’m only resurfacing the damaged portion of the knee — often an area about the size of two fingertips. That means every millimeter matters. Too much bone removed → the implant sits wrong. Too little → the arthritis pain remains. The robotic arm does NOT perform the surgery for me. It works as a guidance system. It places guardrails around the planned cut so I can stay exactly within the safe boundary while preserving normal bone and ligaments. In my technique, I use: • a guided oscillating saw for the flat surfaces • a high-speed burr for fine shaping and form preservation The goal isn’t aggressive bone removal. The goal is removing arthritis while keeping your natural knee mechanics. When the implant fits accurately, the knee often feels more normal than a full replacement — because most of the knee is still yours. Not everyone is a candidate, but for the right patient, a partial knee can be one of the most satisfying surgeries we do. When fibula was preserved during index fusion surgery, a take down fusion and conversion to replacement is possible. 🛑 Motion will not return to normal even with a successful ankle replacement, however it will improve. Pain free ankle is the goal. 🛑 Revision to open fusion with plating or TTC fusion are also valid options. It is a discussion with the patient in regard to risks and benefits to each of those surgeries. 🛑 70 yo patient underwent arthroscopic ankle fusion on a outside hospital, went into a non union and presented because of ankle pain. 🛑step by step takedown fusion with conversion to Inbone ankle replacement #footandankle #orthopaedics #podiatry #patientcare #anklesurgery
