Just watched Peter Attia’s 60 Minutes interview. One of the best explanations of longevity I’ve seen. He talked about something he calls the marginal decade — the final 10 to 15 years of your life when everything starts to decline. Strength, mobility, cognition, independence. At 75, most people fall off a cliff. You see it everywhere — friends, parents, colleagues who were sharp and active one year and barely recognizable a few years later. Attia’s question is simple but uncomfortable: What are you doing now to make those last 10 years worth living? He’s not promising immortality. He’s not selling peptides or the next anti-aging supplement. His prescription is brutally ordinary: → Build strength and muscle. → Improve your VO₂ max. → Prioritize emotional health and relationships. → Do it long before you “need” to. He calls this shift Medicine 3.0 — moving from treating sickness to training for health. The goal isn’t to erase the marginal decade. It’s to make it enjoyable. To stay capable enough to live, not just exist. What I love about this perspective is that it reframes longevity. It’s not about chasing youth or adding years to the calendar. It’s about preserving the ability to do the things that make life meaningful — hiking with your kids, skiing in your 80s, carrying your own bags, remembering your stories. Most people spend decades building wealth, but almost no one builds capacity for their final decade. They wait until decline starts, then try to reverse it with medication or quick fixes.
