EYIn this optic nerve, we see a deep excavation with a vertical component approaching 98%. But this image alone does not “diagnose” glaucoma. Glaucomatous optic neuropathy does not live in a photo — it lives in the context.
What makes this glaucoma is the full construct: a nerve that is actively losing fibers under an ongoing risk factor. The damage occurs at the level of the lamina cribrosa and translates functionally into characteristic visual field defects.
The video adds something essential: movement. It provides a better sense of the optic disc’s three-dimensional structure, avoiding misinterpretation that can happen with a static image.
In glaucoma, we are not looking for a “typical” picture. We are looking for consistency between structure, function, and clinical context.
